Extended Homecare is licensed by the New York State Department of Health to provide Skilled Nursing and therapeutic Home Health Services to clients with Developmental Disabilities, including Acquired Brain Injuries, Neurological, Congenital and Related Behavioral Health Challenges.

Since 1997 Extended Home care has been a special Needs, certified Home health Care Agency. Our main focus is to provide high quality home care services to PWDD patients and their families. Extended Home Care is providing services to all patients with in the five Boroughs of New York City, as well as Nassau and Suffolk County.
To provide nursing services and to teach our patients and caregivers how to manage medical conditions and maximize quality of life in the home. To help our patients regain and maintain activities which affect their daily life in the home. To assist our patients with entitlement/benefits and referrals to available community resources, counseling and crisis intervention.
Extended Home care's Registered Nurses provide skilled nursing at home to patients who require the professional care of a Registered Nurse in order to assess, manage, observe, and evaluate their care. Skilled nursing must be intermittent (not require continuous care), and must not be able to be provided by non professional staff.

3 TERRIBLE therapists from this agency for OT, PT and speech services.
Three specific therapists were very unprofessional , incompetent on my child's needs. THEY WERE either NOT showing up for the scheduled session but still submitting claims to get payed, or showing up 2-3 hours after the scheduled time they provided, when they would show up they were higly unprofessional and caused distress to my child, as they were not competent on her needs.
After requesting change of therapists (agency took very long ) we finally have an AMAZING speech therapist, but no OT and PT as the agency "has a shortage
