Mansura Family Clinic
All Patients have the right to receive treatment and medical services without discrimination based on age, sexual orientation, religion, disability, nationality, sex, color, race, gender identity and/or source of payment. Established in 2006 by Herndon "Buzz" Jeansonne, Louisiana Healthcare Practitioners (LHCP) opened new doors to it's surrounding community by offering quality healthcare close to home.

Previously owned by Dr. Newel Gauthier, the Cottonport Family Clinic and Simmesport Family Clinic were each open for half of a day, making it difficult for those who lacked transportation accessible to healthcare. Once under ownership by Buzz, both the Cottonport Family Clinic and Simmesport Family Clinic expanded its hours of service to full 8 hour days.
Services
Cancer Screenings, including blood test for prostate abnormalities, pap smears, and referrals for colonoscopies and mammograms. Cardiac screening, including laboratory testing, referral for EKG/ECG, and referrals to cardiologist. Many preventive services are covered by insurance plans. You may refer to your insurance handbook or contact your insurance carrier for further information regarding your benefits.
Cottonport Family Clinic is proud to announce it is now offering General Substance Use Disorder Treatment (SUD) and a Medication Assisted Treatment Program (MAT). These programs allow us to provide on-site mental health services such as psychiatric evaluations, group and one-on-one counseling, addiction services, case management, and many others.
To renew your prescription(s) when there are no remaining refills, call your pharmacy directly with each medication that you are needing refilled. The pharmacy will send us a request for the refill, if required, which we will respond to in a timely manner. To avoid missing doses of medication, contact your pharmacy at least 4-5 days before you run out of medication.
It is our policy to collect payments upon service, including any deductible, co - payments and self - pay payments. If your insurance has a co - payment, your insurance carrier requires that we collect it from you at the time of service.

We bill most large insurance carriers as a courtesy of our patients; however, we always suggest you check with your insurance company to verify that our Nurse Practitioners are participating providers in your plan.Below is a list of most insurance carriers that we accept. If you have any questions about your fee or the insurance companies with which we contract, please contact our billing staff at (800) 462-0742 EXT 150.
