Services
Priority Home Care, LLC offers exceptional care from skilled medical professionals in the comfort of your own home. Professional nursing services are provided in accordance with the plan of care and are case managed by licensed Registered Nurses who may also be the visiting nurse assigned.
Additionally, case managers may use LPNs (Licensed Practical Nurses) who service cases under the supervision of the RN and assist in the implementation of the plan of care.Occupational therapists treat disabled, ill or injured patients with special equipment and the therapeutic use of daily activities.

Program provides home and community-based services to eligible persons age 60 or older that are clinically eligible for nursing facility care. Services available include: Adult daily living services, Companion services, Home health care, Home support, Personal care, Respite, and more. Program provides home and community-based services to eligible persons age 21 or older who have symptomatic HIV Disease or AIDS.
