Patient Care Coordination

PCC was inspired by a Social Worker's passion for serving his community and a desire to create an enjoyable workplace for compassionate people who shared his call to serve those in need. Paul Rimar established PCC as a Service Coordination agency in the fall of 2014. Paul began as the Executive Director, Service Coordinator, Administrative Assistant and Marketer.

During this critical time, it is our priority to ensure the health and safety of our participants and dedicated staff. With that said, employees of Patient Care Coordination are working remotely until further notice. Patient Care Coordination remains open and operating during regular business hours to ensure that all our participants have necessary backups in place and ensuring their health and safety in the community.

PCC stands behind its mission and values to ensure that participants are receiving community resources based on proper assessments to meet their individualized needs. PCC stresses the importance of value-based care and how our improved participant outcomes contribute to better population health.

To aid in achieving these objectives, PCC remains active in the community and has partnered with area organizations to assist our participants in accessing and connecting to community resources.PCC works directly with community health partners to ensure greater continuity of care. By working directly with area health systems, we bridge the gap between acute care systems and transitions to the community.

Patient Care Coordination is always looking for talented individuals that can contribute to the continued success and growth of the organization. Patient Care Coordination is proud to have a diverse group of Service Coordinators that represents all stripes of the community we serve.

What are Home and community based services? HCBS provides for supports and services beyond those covered by the Medical Assistance (also referred to as Medicaid) program that enable a person to remain in a community setting rather than being admitted to a Long Term Care Facility. Currently the Department of Human Services administers 12 HCBS programs.
