Services
Primary Prevention Home Care has been in business since 2010. We are a family owned and operated business, treating each patient as a member of our family. Primary Prevention Home Care is a JCAHO accredited organization and accepts Medicare, Medicaid, and private insurances. We serve New London, Tolland, Hartford, Middlesex, and New Haven Counties, with offices in East Hartford and Meriden, Connecticut.
Nursing Services are performed by a registered or licensed nurse. Typically, these services are only provided when; a physician orders this type of care, care must be provided on an intermittent or hourly basis, care is reasonable and necessary for the treatment of the patient's illness or injury, and the care is not custodial in nature.
Homemakers and Companions help an at-home patient with their activities of daily living and with various tasks and chores around the house and community. A homemaker or companion can assist with meal preparation, cleaning, laundry, money management, pet walking, errands, shopping, and transportation to doctor appointments, among other tasks as needed.
HOME HEALTH AIDES: Territories Needed: Norwich, Hartford and Meriden areas RESPONSIBILITIES: Demonstrates professional growth, and competency. Accepts clinical assignments that are consistent with education and competence to care for patients. Provides necessary skills to assist the patient with safe transfers, exercise, and ambulation per agency policy.
