Memory care is designed for seniors with Alzheimer's or other forms of dementia. As Alzheimer's disease or dementia progresses, the level of care and assistance a person requires increases. Many families prefer to keep their loved one home for as long as possible, but at some point a person with Alzheimer's will need more care and safer environment than you can provide at home.
Transitional Care (TC) is a new provider program that was created by Medicare. TC helps you avoid complications and readmission to the hospital by helping you to manage all aspects of your care for 30 days from the time you get home from your inpatient facility. A day or so before your discharge from the facility, your Transitional Care Nurse (TCN)will visit you to make an initial assessment.

In-home healthcare was the best choice we could make for hospice care at home. It is the perfect solution when you can not be with the ones you love. The schedule I set up was 5 days a week in the afternoons (so I could work without worry) - typically 6-8 hours. The home care team made meals, played games, interacted with, and cared for my father. Excellent!

When you need home care in Orange County Care Partners Home Care is definitely one to talk to. They are very knowledgeable and compassionate to your situation. For me, it worked out great. I can use them to manage my parents on-call or with a set schedule…typically 3-4 days per week. I know when they are at the house we have nothing to worry about, they will interact with my parents, and make sure they enjoy themselves - a much better solution than putting them into a nursing home or assisted living.

Personally want to thank the team at Care Partners for providing knowledge, care and support for my mother. The in-home care they provided is exceptional, beyond compare. They are well trained and love working with seniors. In-home health care is a bit more expensive than sending to a group home or assisted living, but it is worth it to have someone at my parents house - where I know they want to stay.

These are some examples of the worst service I've ever experienced. I was in the hospital after surgery. Marco informed me that the caregiver was a no-show, which meant that there was nobody to feed my dog and let him out. Marco finally went and did it personally. Another day when I was at home, nobody showed up.
All the caregivers who did come were very nice and professional.
Randy called me and apologized for the horrible service. He made a substantial adjustment to my bill.
I have no family to help me, and I counted on Care Partners for help during this surgery. They failed.

My family and I were very blessed by the excellent care my mom received in the last month of her life before passing on 5/20/21. She battled metastatic breast cancer and was a warrior in every sense, and the Care Partners was a perfect fit for us. Tifany was fabulous from my very first call to her… she was kind, compassionate and helped us tremendously. Anjanette was my mom’s caretaker and we LOVED her!! She was so gentle, respectful, friendly, and had a great sense of humor! We all looked forward to her visits! My mom loved her and my dad appreciated her company. There are a lot of caretaking

In-home care agencies are a dime a dozen, but every once in awhile you come across a sliver dollar. Care Partners is that silver dollar and undoubtedly the best in in-home care providers. I have been a professional in the senior living industry for more the 13 years and have never come across a home care agency that has the professionalism, culture, passion, and expertise that Care Partner demonstrates daily with this clients, team and community.

This review is related to the customer service at the intake stage. We contacted Care Partners Homecare after a referral from the hospital when we were in need of 24 hour care for our family member who desperately wanted out of her post care placement at a SNF. Josh did a phenomenal job responding timely and providing all the info needed to make the care choices with our family member. While we ultimately needed a different solution, we were impressed with their expediency, responsiveness and professionalism. The social worker at the skilled nursing facility provided me with this company as an option because they had provided excellent care in the past. After interviewing options, I signed the contract with Ben in a matter of hours & arranged 24/7 services for 3 weeks in the skilled nursing facility. The patient had dementia & needed skilled nursing for antibiotics, strength training & COPD care. Overall, the caregivers were a good fit and went above & beyond to do a superior job. Choose another agency. Always getting the schedule incorrect, they promise they will provide quality service this is not when the schedule is always wrong. They don't listen to the client they are scatter brain people. They add to your hours and charge you for them. They changed my contract after a few months, there is a 24 hour in advance change they verbally changed to a 72 hours no paperwork follow up. Just because they use the phrase have a blessed day this should not give you a assurance this agency is good. Their scheduling team assigned two caregivers on the same day. The caregivers

My 89 year old mother had a stroke in April 2018, and she decided that she wanted to return home after spending a few weeks in a skilled nursing facility. My brother and sisters and I checked out a number of different in-home caregiving providers. We selected Care Partners, and they provided my mother with excellent service for over a year until she moved to a new community outside their service area.
For several months Mom required 24/7 care, and Care Partners was able to bring in caregivers who were very skilled in helping her with all of the daily activities that most of us take for granted

My 87 year old father in law recently needed to have some help shopping, cooking & cleaning. Catalina came out for an evaluation & was very patient with him trying to find someone that was a good fit for him. We have used them on several occasions when I am out of town or have had an emergency & cant care for him myself. We have both been very pleased with the care & support provided by Care Partners.
I also appreciate that Catalina checks In with me to see how things are going & if I need help. I would recommend Catalina for anyone looking for care for an elderly family member

Everyone at CarePartners was wonderful to work with in helping my elderly Mom recuperate from a nasty fall. The administrators were always available to answer any questions our family had, and they dealt with problems quickly and effectively. The aides were very caring reliable, and patient. Most were willing to go the extra mile to do something special for Mom, which our family really appreciated.

My mom has been receiving care from Care Partners since her brain injury in May 2017. Care Partners has been amazing. They have always found staffing that mom liked, they have been incredibly flexible and available, and are ready to step in when we need something extra. Additionally, they have counseled us and made recommendations as we needed other resources for mom such as an assisted living facility, or just had questions about the aging and recovery process. As an owner of the business, Randy has stepped in whenever something rose to his level and always made sure that we had a great resolution

This is an agency that really cares about matching the right caregiver with a patient. Rodney, one of the owners who met with us personally, makes sure things are going smoothly once care is in place, and they really try to make sure our scheduling and care needs are taken care of. I would definitely consider Care Partners in your search for home caregiving.
