Basil R. Besh, M.D., is board-certified by the American Board of Orthopaedic Surgery. He specializes in Hand, Wrist, and Elbow conditions, and is one of the few hand surgeons in Silicon Valley that has the Subspecialty Certificate in Surgery of the Hand. Dr. Besh has over 15 years of experience focused on orthopedic hand conditions.
Joshua Ramos, OTR/L, CEAS, is a Registered, Licensed Occupational Therapist and Hand Therapist. He graduated with a Bachelor of Science degree in Health Science in 2012, and a Master of Science degree in Occupational Therapy in 2013, both from the Dominican University of California.
Joshua is also a Certified Ergonomic Assessment Specialist and has performed ergonomic onsite evaluations, allowing companies to provide their employees with a safe workplace and reducing workplace injuries.Additionally, he is currently pursuing his Hand Therapy Certification. At FORM Hand Therapy, Joshua enjoys meeting so many different types of people and hearing their stories.

An evaluation of your condition may include a physical exam and diagnostic testing such as an X-ray or MRI. These services are conveniently located within the facility. The advantages of digital X-rays include less radiation than traditional X-rays and allows for films to be viewed and stored on a computer for immediate availability.
FORM Ortho specializes in the rehabilitation of Hand, Wrist, and Elbow conditions. Our Hand Therapists are experienced and knowledgeable in the intricate anatomy of the hand and upper extremity, advanced surgical techniques being used, and rehabilitation protocols, allowing us to maximize your recovery.

What happened? Poor follow up treatment in the Therapy Dept. with regards to providing the right patient information and their paper work. The Therapists are great like Kevin, Sophia, and Jamie including the Aids but of course they can only provide treatment as they are ordered and approved. Despite proving clerical with the paperwork, they are still confused and erroneously provided the wrong information (ie. being treated for right hand not left to my surprise). This is failure to include patient in on their their own care planning and treatment...mine! This feedback was escalated because

I was directed to Dr. Besh office after seeing another specialist since they did not take my insurance. Before even heading to this location the specialist called Dr. Besh office and was told I can come in right now to be seen right away. I made sure to let them know I needed to be seen right away since I just gave birth and had breast milk on me and I needed to drop it off to my daughter at the NICU in Washington Hospital.
Denisse, at the front desk was kind enough to let me store my breast milk in the fridge. Good thing, because I was nearly waiting over two hours! When I finally got into one

I broke my wrist in November and had a hard time finding a affordable hand therapist in Santa Cruz. My daughter worked at FORM and recommended that I make the drive to Fremont. What a lifesaver. It's kind of a one stop shop for hand therapy with many therapists on site. They are also affordable, charging you only for the visits you make. The therapists are very friendly and knowledgeable. I worked with Sophia, Joshua and Andrew. Great people, great place.

I paid for the bill shortly after my appointment. More than 1 year later, they send another bill for a different amount that was for use of the clinic room the visit occurred in. No other service was provided. It was only a conversation with the doctor.
Bill was paid to make sure it didn't go to collections.
The Doctor was fine, but this establishment needs some help.

Dr. John Jaureguito was thorough, concise, a wealth of information. And he was quick & sincere with my initial visit. From the moment I walked in I felt important. The restroom was clean. There were a couple of new flat screen TV's on but not too loud & plenty of professionally framed sports memorabilia all around their well decorated office with nice soothing earth tones. A clean Keurig coffee machine was filled with H2o in the waiting room with fresh individual creamers, stir sticks & clean hot cups with lids. While my wife is the patient, I feel compelled to write this review. My wife had knee replacement surgery three days ago, and the care that she received was outstanding. I would like to share my perspective as the caregiver.
FORM's staff is kind, competent and committed to helping everyone. I was invited back to sit with my wife after she had been prepared for surgery, and we had 30 minutes together to talk and be together. I was even encouraged to accompany her as we went down the hall to have an x-ray taken.
When she was out of surgery, I was immediately informed that all went great

Yesterday, went to this orthopedic place to do my Open-MRI on my ankle.
Alex(the MRI tech guy) is very nice and polite and efficient.
I am pretty nervous about doing MRI, I Never done it before, but
he make it pretty easy. Just can't move for 45 minutes to get clear-image.
Thanks again Alex. But will see how expensive they "billed" me, I have not
seen my bill yet, maybe I will have a "new update" on the bill.

I've had multiple knee procedures done on both sides of the country, but FORM is the only place I've ever been to that really has their act mastered. They do the imaging, operations and PT all in house, and they are conveniently located. They are fast, courteous, and highly professional. I would and will recommend them to my friends and family.

I come here to see Dr. Wilson for my feet problems. He is an awesome doctor. The first consultation took about 1.5 hours. Dr. Wilson made a very thorough examination of my feet including checking my gait and getting x-rays done. The dx was plantar fasciitis and heel spur on both feet. Ouch!
Dr. Wilson has a conservative approach to treatment, which I truly appreciate. As a test, he recommended that I followed the stretching exercises and wore tape on one foot for one week to see if it would help before proceeding to custom-made orthotics--which could cost time and a lot of money.

I have had surgery on each wrist by Dr. Besh and you can barely see the scars and pain was significantly better after surgery. The staff is always very friendly, they remember my name every time (which is impressive considering how many people they see) and are always helpful. Being an RN I'm always a bit more critical of medical facilities and I have to say I really like FORM. The doctors, the office staff and physical therapy staff are wonderful. In particular, I am grateful to Kim. She is very friendly, very caring, and genuine. She is an asset to that office. I highly recommend Dr. Besh

I had a great experience with the FORM in Fremont. Last month I suffered from a herniated disc at two locations in my lumbar spine. The pain came on unexpectedly and I was unable to sit or walk - this all happened on a Saturday morning and since most doctor's offices are closed on weekends, I had to wait until Monday.

Do not let this doctor (Jaureguito) operate on your knees; he will destroy your life like he did to me at age 49.
Get a second opinion. Do not let him touch you.
In 2009 he guarantee I will be back to normal with 2 weeks, I complain to him every month for 1 year after surgery, finally after 12 months he took x trays, his reply to me is "now you are worse them before surgery for the tear meniscus, now you need knee replacement" at age 49 he destroy my life I am in pain every day need to take pain pills every night, thanks to this doctor who do not care, all he cares is collecting money

I've been here twice now, once for muscle sprain in my back and now for a rotator cuff injury. Dr. Dhanoa is very courteous and takes time to answer my questions. I never feel like I'm being rushed. The front desk staff are very nice as well. The therapy sessions for my back really worked and I actually looked forward to going! If you have any sports injuries or back problems, I would give these guys a go, they certainly helped me.

I really like these guys. I've been to multiple appointments at Dr. Jaureguito's office. Generally things are timely but like most doctors offices they can run a few minutes behind. For me, what is more important is that I see a specialist to help treat my pain or illness. He spends the right amount of time with me to ensure all of my questions are answered. He is very thorough and very friendly. I also can't say more about his staff. They are pretty amazing, friendly, professional, and helpful. I've had my knee operated on my Dr. J and been treated for tennis elbow by Dr.
