We are very excited about this rating and proud of our team!. Vista is located in Pasadena, Texas, near several major acute care hospitals as well as many residential neighborhoods.

Our center is a one hundred-twenty five bed skilled nursing facility which provides twenty-four hour residential health services for persons who are no longer in an acute phase of illness but who continue to require frequent nursing observation, care and rehabilitative services.Opened in 1978, Vista Nursing & Rehabilitation Center has worked over 40 years building and maintaining a first rate reputation as a vital part of the Pasadena community.

Vista Nursing and Rehabilitation Center is here to assist with short-term, long-term or respite needs. Our professionally trained staff are here for one purpose, to serve you. Generally, when someone is in the hospital, they are assigned a case manager or discharge planner who will assist with the transition back to your home setting.
We know that the decision to come to a nursing facility for rehab OR a long term stay can be a taxing time on ones emotions. I'm a taxi driver who picked up an elderly lady with her belongings took her to this location that she wanted to go to after getting her there and getting her in her wheelchair and her belongings all by the front walkway about to leave some clown comes out waving his arms practically yelling at her telling her she cannot stay there telling me to get her out of there the lady almost in tears people coming and going watching I felt sorry for her I told her I had to go on another trip for someone going to the airport and I was running behind but if she needed us to call us back and we would come back

My mom left here after a year with a broken finger and active fear of the staff. It took years to get her past that. Do NOT leave your loved one alone here. Go by daily, take them for a walk outside and ask them every day if they feel abused. And let them know they can move.

On very short notice I desperately needed to find a nursing home that would provide high quality nursing/ hospice care for my Dad. I live out of state so online research was discouraging (one nursing home didn’t answer the phone on weekends!) Vista got better reviews and a higher Medicare rating and I feel so fortunate they had an opening. So far our family has been pleased with the cleanliness of the facility, the care provided by nursing staff and the communication from administrators, nurses and hospice staff.
Please be aware that nursing homes are not allowed to use full length bed rails

My mom fell and hit her head and nothing was done about it. I wanted to visit on Mother's day but they don't allow visiting on Sundays. My dad called me to let me know my mom fell again and hit her head but this time was taken to Bayshore and diagnosed with brain bleed and dr told my dad she had 2 blood clots on her brain. One old and one new. Said if they had to do surgery she probably wouldn't make it through. Patients who are a fall risk should be put in beds with rails to keep them in bed. Moms only had a rail at the head. She had no tv and they never took her out of her room to enjoy activities

Vista took great care of Mom. She loved the social activities, especially Bingo. Mom just passed away. We were treated with care and compassion. You can not ask for a more loving and caring staff.

When looking for a nursing home I wanted my Mom in a beautiful place and I put her at Rolling Brook in Baytown and The building was beautiful but the compassion was definitely lacking. I was asked to pick my mother up the second day she was there. I had to find a place quickly and it had to be a locked facility. Vista had that and took my Mother the next day. The building is old and not what I had envisioned for my beautiful Mom but the care she received was wonderful. Pam, Ken and all their staff were so compassionate and caring. This place is really rundown. It’s loud. It feels extremely dirty. My grandmother has been there only a few days and she acts like she LOVES it. I seriously have no idea how, although the staff seems to be very friendly to her. Have had no problems so far, my review is just going off of the place being so rundown and feeling dirty. I will update my review as time passes if she still loves it. The staff could make it worth it I guess, but I mean there’s no way this place has been updated in the 40 years it’s been open. Vista Continuing Care AND Rehab??? It should read Vista Morgue. My brother entered this place after the rosy picture painted by Pam, Lacy, Tonia and Ken. "Just because they are elderly and have trouble walking, we treat all our patients with dignity and respect". Right. My brother sat in a bed that was so worn his butt was only 6 inches from the floor. His wife brought in all his medicine from home but he didn't get any except warfarin. For 9 days he sat in his room on that filthy bed without a shower, he used the toilet at home and a urinal. At this Vista they put him in a diaper and changed it

After many attempts (5) of placing my mom in a dementia care facility we came across this place. Thank GOODNESS we did. Yes it's an older facility and nothing super fancy but it is always clean and the staff are incredible. My mom seems happy here and her level of care is excellent.

I am a volunteer at Vista and have been going on Tuesdays do the bingo with the residents for over 1 year. I don't know the staff because I just deal with the residents, but I have to say that what I see is kindness. The staff seems to be very kind to the residents. I often ask the residents how they are doing and I've never heard a complaint about the staff. I think it's a good facility. It's very old but gives good care.
