Our job is to provide health related care for individuals in the comfort of their home. It is our mission and has been our focus since 2001 when our organization was formed. We continue to provide the highest quality care from four locations along the front range of Colorado with dedicated and committed staff members.

Under the direction of the client's physician, our nurses provide personalized care developed to best meet each client's needs. We can provide specialized programs for clients living with diagnoses such as diabetes, heart disease and cancer.

At CHHC, we create a rehabilitation program around the specific needs of each client and provide evaluations, treatment and education for individuals who are experiencing decreased mobility due to an illness, injury, stroke or orthopedic surgery. CHHC's team of physical therapists work with the patient and MD to develop a specialized plan of care to improve such areas as strength, endurance and ambulation.
At CHHC we understand the problems a family experiences when a loved one is unable to perform personal tasks such as bathing and grooming, as a result of an injury, medical condition, or simply part of the aging process. CHHC is an experienced provider of personal care services offered in the privacy of home.

I WISH I COULD GIVE THEM ZERO STARS. Their billing department is an absolute joke. Therapists were very kind, but scheduling with them was inconsistent and we had multiple no shows. Therapists told us we had to pay copays in cash which was inconvenient. The main office is incredibly difficult to communicate with—they were an absolute nightmare working with our insurance. We waited on them to submit the correct documentation for services for over a year. I do not recommend this company to anyone.
