Long term care is a variety of services which help meet both the medical and non-medical needs of people with a chronic illness or disability who cannot care for themselves for long periods. Long term care is focused on individualized and coordinated services that promote independence, maximize patients' quality of life, and meet patients' needs over a period of time.
Sometimes we never know what life's going to hand us, especially as it relates to health. There are times when you may need the skilled and advanced medical resources you can find at a hospital but in a home-like setting, such as after you've had joint replacement surgery, experienced a stroke, are dealing with a respiratory illness or Parkinson's disease.
Our short-term rehab at Fountain Bleu is a bridge for those individuals leaving the hospital, but are not quite yet ready to return home. Our highly trained staff is committed to providing the highest quality rehabilitation. Our Center's rehabilitation services include physical therapists, physical therapy assistants, occupational therapists, speech therapists and restorative nursing staff.
Many Americans provide daily assistance to elderly family members or friends so they are still able to live in their homes. This can be a physically and mentally demanding job. Respite care provides relief for the caregiver. It takes some of the burden and stress away, whether it's for a few hours a day or a few days a week.

My mom lived there for a little over a year. We originally chose this facility for Anna's place a secure memory care unit soon to be completed. We were on the list early on, and patiently waited for it's opening only to learn 1 day before that due to her medicaid status my mom would not be able to be in the unit. My hands were tied and rather than disrupt my moms way of life we kept her in the long term care section. I stayed on top of things and made sure her needs were being met. When the covid lockdowns took affect, that's when things went bad. The communication to family was non-existent, I

Please DO NOT send your loved ones here. My step dad was neglected. Fell out the bed multiple times,hitting his head , swollen wrist. Obtained bed sores. Some of his items came up missing. They are only taking people money and not caring or help rehabilitating our loved ones. Please Please Please do not send your loved one here. If you do,don't expect them to come home 😢

If I could give this place zero stars I would. Do not waste your time or money at this neglectful, HORRENDOUS institution. Their new ownership and staff has been rude and extremely unprofessional on several occasions. Words cannot explain how horribly they have treated a loved one and their family. I hate this place.

Yuck! Idk how this place is still opened. I really don’t. Majority of staff is Lazy , food is always cold , dirty, the hallways smell TERRIBLE. They have the patients just sitting there in the hallway sleeping, the coordinator isint nice. I feel bad for every patient in there. Hope they find a good place :(

The place has came a long way since the previous owners...but I know Charles Dunn personally & patient care with wonderful experience is his main concern...this facility is beautiful & smells amazing...and needless to say the staff is off the charts professional & compassionate...and more than anything...the best man to buy the building did so these patients can receive quality care when they deserve it the most...great job lady’s & gentleman...if no one said it yet? Your all simply amazing 😍 😘

This place was the cause of my grandmas death. Her leg was broken somehow and supposedly no one there knows how it happened. We had family in and out all day everyday and the neglect that is happening there the last month or so is absolutely horrifying. There are two or three good aids in the whole facility and no body else including management cares for these people who are suffering daily. The long term unit is constantly short staffed and there are patients not being fed or changed all day long. The nurses and aids are rude and the director of nursing is where most of the problems are starting

Place is always clean. What staff members I've met are nice. But as with al nursing homes they're under staffed. My husband fell out of bed 1 night. It was an hr before they found him on the floor. And he was screaming for help. And even though he hit his head on the night table. It took them 17 hrs before they sent him to the ER. And only because he was seeing double. And had a headache.

The staff here are wonderful and hard-working. The admission team are warm and welcoming (Hilda and Danielle). The facility is clean, updated and well maintained. The atmosphere and ambiance is like a family environment where people are hospitable and encouraging. We are so pleased to have found a place for our Mom to get long term senior care. Thank you Fountain Bleu!
