We are a 99-bed Skilled Nursing Facility specializing in rehabilitation, wound care, IV therapy, and a variety of other skilled nursing services. We are focused on helping our patients have a positive post-acute experience by providing personalized care planning and quality care outcomes. Our beautiful facility allows our staff the opportunity to truly get to know our patients.

Post-hospital extended care is for residents who have been recently released from the hospital and are medically fragile, requiring special services. Our goal is to help our residents in the recuperation process while helping them achieve their maximum capabilities to return home as quickly as possible.
The dementia care team at Vineyard Post Acute is familiar with and understands that when someone is suffering from the progressive stages of memory loss they will need continued support and assistance. We also understand that each case is uniquely different and deserves the utmost attention and care.
Vineyard Post Acute provides gentle daily structure through various activities to stimulate our residents during the course of the day and early evening. Our goal is to provide everyone the opportunity to be as active as they would like to be. My father, aged 95 and bed bound, was transferred by ambulance from the hospital to Vineyard Post-Acute for physical and occupational therapy following a two-and-a-half-week hospital stay.
1. Upon discharge from Vineyard Post-Acute, my father was examined and found to have a bedsore on his right heel and several more that were developing on his back. Vineyard Post-Acute has no record of these skin conditions.
2. Upon arrival, my fatherās personal effects were inventoried by Vineyard Post-Acute but only some of his personal items were included on the list. They neglected to include his glasses

All of the positive reviews are troubling. Vineyard forever changed my husband's and my life when we had to have his 90-year-old mother admitted there because she fell and injured her pelvic bone. Other than that, she was healthy upon entering. She was there for two and a half weeks and came back to live with us. I noticed immediately that she had a chronic cough. All of us also developed COVID symptoms. Before we could obtain an appt to get her tested for COVID, she suddenly died while sitting in her chair. We had to go through the testing process having just lost her. All 3 of us tested positive

I cannot thank the staff at Vineyard, most especially Wangi, for the loving care given to my sister Lois, during her time there, and for the consideration shown to me after she passed. I am overwhelmed. The hospice nurses and Guardians office working with Vineyard were wonderful as well. Thank you all.

At 37 years old I had a major heart attack and have been in recovery for over a month. The first facility I was sent to after being released from the hospital was Vinyard Post Acute. It has been a fabulous experience, the staff is exceptional, working with nurses, PT, and OT has been nothing but a pleasure. The staff is very knowledgeable and willing to help. Being the youngest patient by many years I have felt like a VIP with my needs quickly being taken care of. My wound care nurse is a Godsend dressing and redressing my wounds which has helped to speed the healing time. The food is probably

I have been in other facilities before and vineyard is the best place to go work. The atmosphere here is wonderful my co-workers are very friendly and respectful to each other. I wouldnāt change anything if I have choose a place to bring a family member this is the place I will choose the care here is fantastic !!! I know I work here!!!!

As an employee of Vineyard Post Acute I have seen a lot of positive changes in the last year. The employees are great to work with and have a positive attitude and are always striving to make sure the residents have the best care possible. The operations manager is always thinking of different ways of keeping the staff happy and makes sure he does his rounds to the residents to ensure their needs are taken care of.

I have had the best experience ever! If you have to be in a skilled nursing facility, this is the place to be. The staff is exceptional, the rooms spacious, and the food is decent. There is a beautiful patio in back and plenty of room to sit back and heal. The physical therapy is grueling but without the wonderful therapists who definitely know their job I wouldn't be getting around independently as I do. All I can say is thank goodness for this facility.
