This is the most depressing place to take your elderly. My father was there for a short rehab period, and when home in a horrible shape. Hungry, weigh loss, and in same or worse condition as he arrived. He didn't even have a diaper on, and smelled like urine. His hair was never washed. Don't take your family there. Front desk staff is rude when visitors come in, or responding the phone. The insurance provider and Medicaid should be aware of the services at this place. DON'T TAKE YOUR LOVE ONE THERE PLEASE.

Beware of this facility. My cousin was here for six days and left there with a catastrophic brain injury. On the sixth day he was there he complained of shortness of breath and asked for help. When help finally arrived he had no heartbeat and was not breathing. He was resuscitated and was transferred to St Luke's Sugarland. His family was never told how long their loved one had to wait from the time he cried out for help and when help actually arrived. Sadly only days later they were told he had suffered brain death. If staff had responded and prioritized as they should have he would still be alive

I’ve been working at Paradigm at First Colony for two years and six months . I always had a heart for caring for the residents and patients to help meet their needs.I have work in other Nursing Facilities as a CNA and Oak Bend Medical Center as a CNA in the Med Surge department ,Telemetry department and Skill Nursing department . As I worked in the Skilled unit at the hospital the opportunity to become a Activity Director was presented to me from the Director of Nursing .Thank you Paradigm at First Colony for giving me the opportunity to continue my working skills .

My sister was only a residence at this facility since January 2021, and we are transferring her to another facility. Paradigm does not have a trained Wound Care Specialist. My sister developed an infection from improper care of her breast. I can commend the physical therapy at Paradigm. They had my sister moving around daily. Now we have a major set back.

Good management and a caring staff is what sets Paradigm apart. We would use Paradigm without hesitation if the need arose again.
Paradigm staff is friendly and kind to their patients. The facility was always clean, patients appeared to be well cared for and meals were generally good.
Our family member was recovering from a pelvic hair line fracture and was admitted in a wheelchair, weak, unable to walk, confused and depressed. Upon arrival medical staff reviewed all medications and determined that one contributing factor patient was over-medication.

Mom had to go here, in order to finish some medication. The first night, her sugar dropped low and if i hadn't been there, i would have lost her. The nurses were ill prepared to handle any emergency. The director/ person that signed her in had the attitude, "Now we got her, we are keeping her." I made sure that they knew, once the medication was finished, my Mom was leaving. I got it in writing as well. I hate places like this, since i was told, so many children seemingly, threw their parents away.

My grandmother went in for rehab after being in the hospital for about a week. She went in well able to move her around with little assistance. She left this facility with a stage 2 bedsore on her bottom, and a HORRIBLE diaper rash. Why did she even have a diaper on is beyond me! She is a dialysis patient so she doesn’t produce urine, and she incontinence! She has lost so much weight over the time spent because she wasn’t eating and no one informed us, neither did they do anything on their end. She’s not able to move any limbs, her strength is GONE! It’s much more but I’ll save that for

Horrible Customer Service, this facility really surprised me the staff is utterly rude I honestly do not understand how they even have jobs. I work in the medical field myself and I cannot imagine a patient having to deal with the rudeness that I dealt with. Not only did I get the run around when trying to speak to a supervisor I got hung up on twice. If this is the treatment for someone who works in the medical field themselves then I highly recommend to stay clear from this facility. For a facility stating they provide exceptional care there is nothing exceptional about there customer service

I am very satisfied since Paradigm has become owner/ manager of the home. My brother has been a resident for over 7 years, and I can see positive changes already in this short period of time. Safety and other important concerns are obviously being upgraded for the better. More attention to the needs of the patient is evident. I am pleased.
