Services
Our social service team assists patients and their families during transitional periods. We also provide essential information, manage requests and concerns, help in care planning and discharge planning for each patient. Assistance with placement to long term care facilities, assisted living facilities and independent living facilities.
Chelsea Gardens is proud to announce that we now offer outpatient physical therapy, occupational therapy and speech therapy at our facility. These programs are designed to restore the patients highest possible level of functional independence through individualized treatment plans approved by your physician.
Our speech therapists provide a continuum of services including assessment, education and treatment for language, voice, articulation and fluency disorders. Our treatment programs are directed towards improving communication skills and safe swallowing. We re-train patients who struggle with comprehension, memory, word-finding difficulties, processing and expression abilities.

My dad had surgery on his shoulder and ankle due to an infection. CHELSEA gardens was suggested for rehabilitation. The first night the nurses were rude and rough. The next day was better for rehab and looked more positive. But when the night shift arrived it started again. The nurses were using his arm that just had surgery to try and roll him over or they would roll him over onto that shoulder. All along he was telling them it hurts and to stop. They would not stop. He was in so much pain from his shoulder and ankle that he couldn't get out of bed.

I am very appreciative for the staff that they have there. They held a special breast cancer survivor tribute for my grandmother. My family was so thankful and I could really feel the love that the staff gave to their patients. My grandma loves her nurses, the food and raves over the soup lol. She says she wishes there was more action, but I’m not sure what kind of action a 98 year old is looking for 😂 anyways they are doing a great job and at that I’m sure it’s a hard one too. This is one of the worst nursing homes I have experience,my husband was there for one week ,he fell and busted his eye and his arm was burnt really don't don't know how his arm got burned ,he ended up in the hospital then they would not accept back in their facility,if I could give this place a zero I would,none caring staff all around,no one never called to check on him after getting hurt so bad on their watch

Chelsea Gardens has been very helpful in helping my 74 year old father rehab to the best of his ability. I don't believe anyone is truly comfortable with turning their vulnerable loved ones over to the hands of a nursing facility, but I was pleased. I will say that I was very hands on and present which I believe plays a huge part in treatment. For the most part, I feel that my father was well taken care by the nurses and cnas. I would like to really commend nurse Olah for being so attentive, personable with lots of energy. CNA, Shaquille did a great job in keeping my father comfortable. Our experience

My grandmother stayed here for five days as a temporary stay. From day one, there were issues. First, when I visited her, I was let inside without signing in and I wasn't screened for COVID either. My grandmother has stayed at other facilities where I was asked for my driver's license and given a sticker or tag for security protocols. CG should implement a system like this to keep everyone safe and the fact that I wasn't asked to sign in made me a little worried about my grandmother's safety while she stayed here. I was only asked to sign in and answer COVID questions plus a temperature check on

If I could give this facility zero stars I would. Upon calling the facility I encountered a rude receptionist who would benefit from training on soft skills. Next after a 6 day stay, my father determined he was not receiving high level care and wanted to check out. Chelsea Gardens tried everything in their power to keep him from leaving to include...
1. Sending a team of people in shifts to persuade him to stay.
2. Refusing to exchange medical information with the home health service.
3. Asking the home health service to persuade my father to stay longer in exchange for request medical information

My MIL was there 6 weeks. People were nice, she loved the PT staff, food ok, probably typical. They were open about letting us come and go. Residents were out in the halls & dining room which is a good thing, not trying to hide them. My MIL had a good attitude about recovering and her PT, and she is very friendly, so that influenced how people treated her and her experience. Even though no place is perfect, she felt cared for, it was clean, people were friendly. If she needed care again, she would definitely go back.
