If you're a new client you can fill out the new client from ahead of time. For each new patient you can fill out the new patient form. We will need a copy of your pets medical history and vaccination records if they have been at a veterinarian previously. Just let us know who provided your pet's prior healthcare and we can have the records transferred.
The surgical service at Leslie Veterinary Hospital is popular, so appointments are required. When you make your appointment, keep in mind for most procedures we admit patients between 8:00 and 8:30AM. The admission period may vary from time to time depending on the procedures and doctor availability.
