Our campus, which is located in Swanton, Ohio, has a country setting with beautiful courtyards surrounded by 16 assisted living suites, 68 skilled or long term care accommodations including private, semi-private and two room suites.

In 1964, Cecil Mitchell established his first nursing care residence. His care and dedication continued over the next four decades by owning and managing several facilities in Northwest Ohio. In 1985 at the age of 67, Cecil Mitchell built Swanton Health Care and Retirement Center.

Assisted living at Swanton Health Care and Retirement Center offers a unique blend of wellness, independence, dignity and quality of life. Is frequently required after an acute care hospitalization and/or surgical procedure. You will receive excellent skilled nursing care provided by a dynamic team including licensed nurses, state tested nursing assistants, registered dietitian with regular physician rounding. Physicians/providers on-call 24 hours a day with physicians/providers in-house on a regular basis.

Stayed 1 night after my knee replacement and walked out. was given meds at hospital to make my bowels move and when they did, it was a mess. was told i couldnt take a sower because it wasnt my day to shower. poopfrom one end to the other with an open 8 inch wound. never slept. horrible place. would not send a gold fish there

Wouldn't let us see our Grandma vis compassion visits who died alone. Staff is low, medical orders, care, and call light response is slow. Maybe the director needs to take a pay cut to sustain a reasonable staff level. Some aids and nurses were great, they are all overworked. Don't send your family here.

I have worked in nursing facilities as an admissions director for 12 years and I have been in every facility in northwest Ohio as an EMT. Swanton Health Care & Retirement Center is truly what a caring, family-owned facility should be: they put patients first, the food is amazing (which is SO important for healing), and the owner maintains a constant presence, so any issues go straight to the top and are resolved. The staff have longevity and are happy to do their work. I have NEVER worked somewhere where skilled nursing and assisted living care residents are truly treated like family.
