I can't say enough about the staff at Grace. They are our second family, and Mom feels at home there. Everyone is friendly, caring, loving and respectful. The wonderful care that is given to our mom allows me to have the piece of mind. I am able to leave after each visit and make the hour and half drive back home, knowing that she is treated like family, and well cared for.

If your goal is to return home quickly, Grace Health & Rehab Center has just the solution for you! Our experienced staff offers in-house physical, occupational and speech therapy for those with orthopedic, neurological and other diagnoses. In addition to physical strength, range of motion, balance and activities of daily living, our team of therapists will address cognitive, memory, social and emotional needs that derive from injury or illness.
Incorporating the latest and most advanced research and techniques, our Memory Care Community is designed to have a home-like feel to create comfort and familiarity while maintaining a safe, social and secure environment. From secure entry doors and soothing colors throughout to outdoor patio areas and walking paths, our residents' safety, physical and psychological needs are our primary concern.
Working as a team of compassionate professionals, our staff formulates and updates personalized resident care plans, ensuring that each resident receives medical attention specific to physician orders while maximizing well-being and comfort. The nursing staff has the primary responsibility for day-to-day care.
Medicare is a federal health insurance program for people age 65 or older and people under age 65 with certain disabilities. Medicaid is a state-administered health insurance program that covers long-term care for low-income elderly and disabled people. The full cost of services (room, board, meals, medication, etc.) may be paid entirely from private funds.

I worked for grace for 2 years. At first it seemed like a good place to work. Slowly but surely for sure, you’ll learn otherwise. Management shows favoritism, see what they want to see on them cameras, listen to what nurses and other aides say about a worker and judge you like that. They seem to terminate good workers and believe words of the WORST workers they got. I know for a fact when I was there, I worked my behind off and the thanks I get was to get terminated. I got wrote up left and right for stuff they “assumed” me of doing and the ones that were actually doing it, they get away

I went through a hurricane that completely destroyed our own nursing home in Lake Charles and God blessed me and a bunch of other residents, to our sister home here at Grace. We've been here since September of last year and they took us in with open arms. We will be going home soon and I wish I could take everyone here home with me. I have made good friends here that I will never forget. I love you all and just know that you all have made an impact on my heart and soul. God Bless you all, Esther

Yes I would. I've seen the video's of how the employees & residents have love, care, & enjoying being together. I wish that there was more people in this world like the employees there.

My mother was a resident for five years and had the best care from the wonderful staff. To see smiles daily on Grace’s residents was so obvious on how they were well taken care of. They are always offering fun games and entertainment continually. I would recommend Grace to all.

The reason why I don't suggest Grace rehabilitation center is because the nurses and staff are lazy and they have no respect or compassion towards the residents... they only show up for the paycheck.. the place is unhealthy and unsanitary.. the place needs to be investigated and shut down ... I have seen this stuff going on with my own eye's...

My mom was there for 6 years. She was Always well taken care of and always enjoyed being there. The staff there are caring, compassionate and fun people. I will always remember you guys and would refer your place to anyone looking. XXOO

My mom was in there in January 2016. She already had thrush, Merse and kidney failure before going to Grace rehab. Hospice came there towards the end. She ended up staying in Grace until she passed. They treated her real good. I would highly recommend this place.

Share the experience, I did at Grace. I made some of the best friends of my life. From administration, staff, physical therapist, an the precious residents. I had a pretty bad experience after my surgery. Therefore I had to take physical therapy. I was very lost an confused at first. Then with the help of my nurses, CNA’s (my special angels on… 5th floor) an my room mate an started therapy I can up an out of my shell. I can say it probably saved my life. Thank You Grace Health An Rehab for being there for me when I needed it bad. Much love sent to all until I can walk. Ack in to see
