Genesis Rehab Services is committed to fighting disabilities by providing an individualized, compassionate, and comprehensive evaluation by educating and assisting our patients in living an active lifestyle that is free from pain and physical limitations. We are a privately owned Outpatient Physical therapy clinic, which offers the latest techniques and treatments from highly qualified therapists.
For your convenience we have provided to you the forms and the paper work that maybe required for your initial evaluation. Apart from the patient intake forms that are separated out with location we have also provided a link for you to print out your customized self reported functional outcome measurement scales.

Lesa is a licensed Physical Therapist Assistant, graduating in 2012 from Ivy Tech Community College with High Honors. She started her career in Pediatrics serving low income children in Illinois and Indiana. She has practiced in a variety of settings to include home health and outpatient with concentrations in manual therapy.
