Thanks for providing eye doctor services to our facility! We sometimes have issues with other providers but always talk about how we can count on you guys. We have a large campus (17 acres) and 4 separate buildings 204 residents. Dr. Cusumano is just wonderful, he goes to the difficult buildings, the residents are pleased with the services.

He is readily available if our nurses have to call him [for] reorder clarification. I have recommended your services! We take the time to schedule regular monthly visits at your facility, at your convenience. However many patients require our services, we will still come to you, regardless of age, mental status or immobility.

Of all the senses, most people believe sight is the most precious. Yet, functional blindness is a grim reality for 17% of all nursing home residents. Another 19% have lesser forms of visual impairment. Many of these cases could have been helped with regular professional eye care. Most residents of long term care facilities are unable to walk or drive and cannot comfortably leave the facility for eye care.
Accurate MDS assessment of visual acuities would be documented for those who cannot communicate. Eye exams and eye glasses covered under Medicare Part B, Medicaid, and many other forms of insurance and HMOs. Our eye doctors can diagnose and treat eye diseases even to the incomprehensible individuals.
Our mobile team focuses on serving the eye care needs of individuals in nursing homes, long term care, and short term rehabilitation facilities throughout Florida.

Because good vision is integral to a high quality of life, we have made it our mission to preserve human sight in the elderly and disabled population by removing the barrier of transportation which often makes it difficult or impossible for some individuals to receive a traditional eye exam.We use state of the art equipment which can be set up in a common area or can even be used bedside if necessary.
