Pontchartrain Health Care Centre is dedicated to providing specialized rehabilitation and skilled nursing services in a caring, comfortable, and clean environment. Our "Independence Day" Rehabilitation Unit, located in an exclusive part of the facility, is designed for those discharged from the hospital but require short-term skilled rehabilitation and nursing services for a successful transition back home.
Our team consists of highly trained rehabilitation and nursing staff with a focus on short-term transitional goals as well as long-term quality of life needs for permanent placement. We believe the best outcomes in healthcare occur when you care about individuals, just as much as you provide the needed services for them.

Services
Our highly skilled rehabilitation staff includes dedicated Physical, Occupational, and Speech therapists. Our rehabilitation model is based on a providing more therapy per day and more days per week to expedite your transition back home. It is a well-rounded rehabilitation program based in individual outcome-driven treatments delivered up to 6 days per week.

Mom went to facility to get an application for residency. She was provided an application for employment. Returned package to reception desk but couldn’t get an application for residency. Called twice: 1st time was told that we needed to get a referral from PCP before we received an application for residence.
Called PCP and was told that we need to get a referral before gettin an application and she told us that we were given incorrect information.
Called back to nursing home and the phone just rang. Hung up and called again later and asked to speak to Admin. Admin is new, she told me that

My mom was there for one, yes 1, night. She was recovering from a stroke and needed physical and occupational therapy. I was against sending her here initially because my mother has a disdain for nursing homes in general, but St. Tammany Hospital insisted she go here (later I found out reason because they both have backend deals with one another). Anyway... First they dropped my mom in the bathroom, damaging her knees even further. Then their way of therapy was to leave my mother siting in a wheelchair for over 3 hrs. This is one of the most poorly run facilities I have ever seen. A good chunk of the staff (Not all of them) are nearly impossible to deal with and they also do not know how to follow protocols involving a variety of different things (HIPPA, DNR Orders, Power Of Attorney, and so on) I do not recommend placing your loved one here. If you want to have a peace of mind when placing your loved one in a facility, this is NOT the one you want to choose.
