EKG is done to find the cause of unexplained chest pain or pressure and also to find the cause of symptoms of heart disease. I have been a patient of Dr. Farizani for years, she has taken care of me since I was a kid. My parents are comfortable with her as well since she speaks in farsi and makes it a pleasant and warm experience for us. She is caring and is knowledgeable about healthcare and has helped me maintaing my Thyroid condition.
Chronic Care Management (CCM) services are non-face-to-face services provided to Medicare beneficiaries who have multiple significant chronic conditions.
In addition to office visits and other face-to-face encounters, these services include communication with the patient and other health professional for care coordination (both electronically and by phone), medication management, and 24 hours a day accessibility for patients and any care providers (physicians or other clinical staff).

I wouldn't really recommend this clinic because (this is a hefty read so beware!):
1) The staff is very unprofessional and disorganized
2) I'm not sure if whoever sends the samples to LABCORP knows how to do their job correctly- that is, which is sending the samples to the lab (well packaged and all) and getting the results. Every time I would send in a sample they would somehow mess up in packaging it and the lab would give me inconclusive results, or my results were incorrect (I would get a second and third opinion at other clinics).
3) I've been misdiagnosed TWICE despite giving clear explanations

Unfortunately, this clinic could not give me the quality patient experience I seek. From the start, the receptionist was curt and unkind. I pre-submitted my new patient paperwork on a previous day and arrived to my appointment 10 minutes early. I was not called back until 35 minutes later. I observed one of the doctors cough without covering her mouth, the same doctor entered the small, confined patient room with her mask on her *chin* (a health hazard to me and her future patients), my urine sample was labeled with the wrong name, my male physician was unwilling to complete my pap smear and called

Dr Farizani has been my primary care physician for over 7 years now and she has always had my best interest in mind. I trust her advice and recommendations more than I have any other doctor I’ve had. I have even moved since first getting her as my doctor and even though I have to drive across town to have a doctor visit I still wouldn’t change from her

All 4 of my children have been seen by the providers at this location and was very pleased with the services provided. I have recommended this clinic to other family members and friends.Keep up with the good work and thank you especially now with the covid -19 situation. I like the fact that you offer telehealth as an option to consult with the doctors without the need to go to the office. Unfortunately people who are positive for that do not respect the fact that they cannot be out and about and still go to public & clinics risking everyone else with no care at all!
