Ms. Bertke graduated from The University of Toledo with a Bachelor of Science in Nursing. She then continued her education at Walden University, where she successfully earned a Master of Science in Nursing and her certification as a Family Nurse Practitioner. Ms. Bertke provides care for patients of all ages, and has been a Certified Nurse Practitioner at Family Health Services since 2015.
You first need to fill out our New Patient Packet. This can be downloaded from our Forms page or can be acquired at the FHSEC office you would like to attend. We offer a sliding fee scale at Family Health Services. The application is available here. All self-pay patients are requested to make a $12 payment at the time of their office visit.
Family Health Services is now offering our patients easy and private access to your medical information online, so you can view your personal health record whenever and wherever you have access to the Internet. How do I access the Patient Portal? To gain access to our secure server on the Patient Portal and become web-enabled, simply sign up by providing us with a personal (non-work) e-mail address.

Been coming here since they first started and I love my doctor, she listens and doesn't automatically prescribe something without running tests and actually knowing if that's what is wrong before governing you meds. My last doctor would be like "sounds like or I think it's this" so I'm going to give you this med and see if it works. The staff is friendly and professional.

I have been going to Family Health Services for quite some time now. I have had nothing but good service since going there. The resident doctors are very knowledgeable and are backed by full fledged doctors whenever advice is needed. Family Health Services also bills their patients on a sliding scale, which is wonderful for those without much money. I have no complaints, I have nothing but kudos to shower them with. I will continue to go to Family Health Services. They have ALWAYS done right by me.

Liked this place at first. I dont like that they make you come back several times to do anything so they can get separate office visits. They wont do 2 things in 1 visit. They had my husband come in several times to pull a tooth they gave him the numbing medicine then decided they wasnt going to pull it and wanted to send him somewhere else. Their practices are not very good.
