San Simeon by the Sound Center for Nursing and Rehabilitation is a community-based not-for-profit, 120-bed skilled nursing, rehabilitation and adult day care facility located on the storied North Fork of Long Island in the picturesque hamlet of Greenport, New York, along the shores of the Long Island Sound.
To protect the well-being of residents, staff, and visitors, the following all-hazards Comprehensive Emergency Management Plan (CEMP) has been developed and includes considerations necessary to satisfy the requirements for a Pandemic Emergency Plan (PEP).
Appendix K of the CEMP has been adjusted to meet the needs of the PEP and will also provide facilities a form to post for the public on the facility's website, and to provide immediately upon request.The CEMP is informed by the conduct of facility-based and community-based risk assessments and pre- disaster collaboration with State and local health departments, emergency management services and local authorities.

I talked to my grandmother on the phone and she sounded abnormally awful and said she stopped eating and that she felt like she was burdening the staff whenever she needed help. I knew I needed to get there to bring her some food for her strict diet after hearing how she sounded. Unfortunately the day I was on my way there, we found out she died. We are extremely disappointed we weren't notified that my grandmother was dying in this institution during her rehab. I knew she wasn't doing well, but had no idea she was in that kind of shape, and I'm appalled the staff did not notify her family. The

The Nursing Director is very Rude. I use to work at this cesspool. Patients get treated like tools instead of people. I called recently to retrieve something and like typical New Yorkers I was spoken to like trash. If you hate your family send them here. If you live then take care of them your self.
