The St. Johns County Welfare Federation dba BayView Healthcare, is a 501(c)3 charitable and health care provider founded in 1920 to "take care of the sick and indigent person in the City of St. Augustine and the county of St. Johns." BayView Healthcare has a long-standing history of providing long term care of the sick and frail, elderly and the indigent residents of the community.

We offer a comprehensive umbrella of healthcare services. These healthcare services allow residents to age in place with the option of selecting the combination of services that best fit their needs now and in the future. Incorporating our professional staff, as liaisons between family and the healthcare needs of each client, we offer security and 24-hour availability to reduce stress and costs to the clients.

BayView Assisted Living at the Pavilion is a full-service, assisted living community in the heart of Saint Augustine.
BayView Healthcare is committed to the safety, health and welfare of our residents and staff. We continue to monitor closely Hurricane Isaias and implementing our Florida Agency for Health Care Administration approved Comprehensive Emergency Management Plan preparation protocols. We are in communication with the St John County's Emergency Management Division FLASH reporting and monitoring system of updates regarding the hurricane's path.

My mother was in Bayview for two months for skilled nursing and rehab after surgery. The care from the CNA‘s and nurses was terrible! They were rude , did not answer the call buttons, they said they’d be back with another CNA to assist with toileting and took 45 minutes to an hour. Shame on you!
The physical therapy however was excellent!
My mother recently had another surgery and is now in Brooks Bartram Crossing for skilled nursing and rehabilitation and the care there is leaps and bounds above Bayview. I’m not sure if it’s because the pay is better and better qualified people are working

If you are an African American you may not want to choose the Pavillion for the assisted living for your loved one because Teresa will definitely conspire to get rid of them. She called the police and EMT on my brother twice and they refused to take him away and informed me that she does this often with people of color. I had left her numerous of messages to get my brother's belongings and she never returned my calls. She is very unprofessional and she does not exhibit the qualities and traits that is needed for humanity in this stage of their lives

My mom was a patient in the rehab for 6 long nights. The therapy was good, however, all other services were very disappointing.

Very clean and neat. It's more like a hotel than a living facility. Staff was very helpful. I led an impromptu holiday sing-along in the lobby in which several residents participated and enjoyed very much. The people who live there seemed to be happy with the place and staff

Pretty place. Clean place. Super fatty super cheap super salty and sugar-e food. HORRIBLE FOOD for anyone.....especially for Seniors. VERY NICE STAFF. most staff need better coaching on medical needs....or need not get involved.

Not impressed. My mom was there for only 4 days. Contracted E coli while she was there. The staff did not seem to have any compassion or have the ability to help. I would not recommend the place at all.

Good rehabilitation center for physical disabilities, accidents to medical issues. Majority of the staff are good. As with any place or business there are those few that are lacking. Overall the facility and staff are great.

This place doesnt even deserve a Star!! My aunt spent a week there and the entire staff was garbage, the Physical Therapists were good, but the NP in charge was a joke!! When confronted by a family member of mine, she walked away, because she Did Not know the answer, how she passed Med School is beyond me!! Do Not!! I repeat, Do Not send your family members to this Dump!! P.S. they changed the sheets once, during her 9 day stay!!

This place is Trash! I experienced up close and personal how the staff treat the elderly living in the facility. They push the guests and patients around like their worthless and talk down to them like their infants. Don’t even get me started with the maintenance staff. I saw them unwelcomly invite themselves into patients rooms pushing and acting like they were even there... it’s quite pathetic. The reason I know all this is because I worked as a maintenance man for them for almost a year. I saw wat wen on behind closed doors.... and it’s Not pretty. If u care about ur loved ones then Ull
