Heritage Park Healthcare & Rehabilitative Services is a modern, state-of-the-art care center, conveniently located in Roy, Utah on 5600 South (UT-97), a few blocks west of I-15. We feature 176 beds and beautiful spacious rooms in both private and semi-private settings. I worked here during the outbreak last year. When I was working Hall A. We had a resident that was complaint about stomach issues. And you can tell he was in a lot of pain. He wanted to go to the ER. Well when we notified the nurse. She said "oh he's such a drama queen" and purposely waited to contact the Dr to get him to the ER. By the time she did, he ended up passing away from whatever was wrong with his stomach.
I wouldn't ever bring my loved ones here.

I've had a great experience here at Heritage Park. When I arrived I was extremely sick and wasn't able to do much. With the help of the CNA's, Respiratory Therapist's, Nurse's, Physical Therapist's and Occupational Therapist's I've made a remarkable recovery. The food is exceptional, the rooms are nice and cozy, it was an all around good experience.

I had to put my mom at heritage a few years ago I was so worried about her but the people there were absolutely amazing, they take excellent care of her. The pandemic has been terrible worrying about her but she has recieved excellent credit Kelly at the front desk is amazing she has helped me so much and Lynn Del has been amazing with window visits, inside visits she is a very caring and awesome lady. And nurse Mary who was on A floor alot when my mom was in that floor she did her hair and always made her look nice. I want to thank everone that has been involved with taking such wonderful care

I am a fulltime caregiver for my father, I spend everyday with him!! My father has been here at heritage park since April 20th 2018! We were flown in on a medical flight from Washington state. My dad is a very high level of care since his stroke 4years ago. Dialysis, trach Patient. we had been to 5 differnt skilled nursing homes up in the Pacific northwest. And I can now say how so very blessed for this move to Roy! Yes when you 1st walk in the building seem out dated. I have seen amazing changes since 2018! But more importantly this is my family now! They have done so much for my father and myself

For starters I had to rate this place one star just to leave a review, and they dont deserve that. One of my dearest friends just left there after a major hip surgery. She was suppose to stay there for a total of 21 days and only stayed there for 9 days, because of the care that she received was so atrocious. When I first went to visit her I was confused because I thought I was at the wrong place because this place looks like it was condemned. When she first arrived at this horrible place she went without her pain medication for over 12 hours, after just having a major hip surgery. Her hip was

I love this building. I have personally seen for years the love and dedication from this staff. This building has fantastic leaders smart and dedicated aides and nurses. The food is fantastic, the activity department is known around the community for being the best. So glad we have a Skilled Nursing center like Heritage Park in our community. I absolutely love this building!!

I love my apartment! A beautiful large open kitchen with lots of storage! That master bedroom is wonderful. If I could change anything it would be to make the 2nd bedroom a little larger, add a linen closet or more shelves in the laundry room. The staff here is amazing and maintenance is quick and efficient! It’s a great place to live!
