Services
The Silvercrest Center opened in 1989 under the sponsorship of Booth Memorial Medical Center and The Salvation Army. Originally named Booth Silvercrest, it took its "first" name from the medical center and the founder of the Salvation Army, General William Booth. The name "Silvercrest" came from the silver crest found on The Salvation Army uniforms.
As an inpatient, post-acute rehabilitation patient at The Silvercrest Center, you will be cared for by an interdisciplinary team of exceptionally trained staff at all levels of care, supervised by an on-site, board certified physician. Our 24/7 skilled nursing care model is RN-driven, which means our staff consists of 80 percent RNs with 90 percent of those holding a Bachelor of Science in Nursing degree.
For a wide range of outpatient rehabilitative care, The Silvercrest Center provides customized rehabilitation plans, continuity of care with one-on-one sessions with a qualified therapist every time, and an interdisciplinary team strategy, all of which work cohesively together to maximize best health outcomes.
The Silvercrest Center is primarily a place to rehabilitate, whether as a post-acute inpatient who needs intense rehabilitation after a hospital stay and before going home, or as an outpatient rehabilitation patient who requires follow-up rehabilitation to maintain strength and function to avoid re-hospitalizations.

Like other post if you love your family dont send them here. My father has stage four cancer and the hosptial he was in sent him to this nursing home. He has no movement on his left side of his body due to the tumor. The nurse there are unresponsive when you want answers. They seem to have no heart for any of the patient's. My father has to crawl to the bathroom with none of the nursing assistant. This place should reconsider management and employees .

Therapy is great, morning nurse Debbie is a people person, listen to you needs. Nursing Assistant Churna is awesome. Don't even consider your dietary needs, food is always cold and salty. .never mind if you're on a restricted diet, you are given anything . Not the best place for someone who can't advocate for themselves and not the safest place either . I was sent here from New York Presbyterian if I had a choice I wouldn't but it was worth my therapy sessions. This place needs to be better managed.Three star because of therapy.

My Sister was transferred there for rehabilitation. I was not given correct information as to visitation. Spoke with Director of Nursing. I am optimistic that this will be addressed. There needs to be updated factual information on their website. Family needs to be orientated to the facility's policy. Care should be competent, wholistic and empathetic in approach. I remain optimistic.

Ever since covid-19 began, this facility has taken every opportunity to put up roadblocks in contacting and visiting residents. The staff is quite often combative and rude, and enforce "state guidelines" on a whim whenever it's convenient for them in order to deny various requests. I regret having my parent in this location.

Horrible place, IF YOU LOVE YOUR FAMILY DO NOT PUT THEM HERE! My grandma was there for 2 weeks and has bruises and cuts all over her. She went in just for rehab 2 weeks ago and they have her so drugged up she didn’t know who I was. She was sharp as a knife before she got there and went just to learn how to walk after surgery.

Hurendous I wouldn't send my worst enemy there. The staff are rude, unprofessional ,uncarung. My friend went there fir rehab after minor surgery for rehab she received no rehab, they didn't allow phones, no way of getting intouch with her they didn't allow visitors and wound up getting exttreem infection and had to be rushed to hospital. They also drugged her up on zoloft a drug she never took in her life they should be shut down

Awful awful place. The nursing is horrible im supposed to get pain meds every 3 hours ive waited ove 15 hours at times to get it. The place is filthy and smells horrible. I must reiterate how lazy, unresponsive, and useless the nursing staff is. The dr. Never shows up,the dietary makes things up as they go along and the food if they give it to you is abysmal. I dont like to give negative reviews but this place is just that terrible. If you have a loved one who needs rehabilitation DONT SEND THEM HERE.

If you have a better option I would suggest you take your family member elsewhere. The lack of follow-up on phone calls made this facility is horrible. This is the epitome of apathy. Several times I made phone calls nobody ever picks up the phone at the nurses station. I leave voicemails for a call back I don't get responses. New York State shut this facility down now!

Silvercrest is the worst nursing home. Do not put your love ones there.
They never answer their phones. They don't clean patient. the place is under Staff. My mother was never clean every time i got there i had to beg them and help them. they do not ck on residents. They cause UTI many time. She end up in hospital due to their negligent. Physical therepist never got my mother up from the bed and she keep getting sick.
when they call 7:30 saying we are calling 911. She passed away 8
when i got there she was dirty. Staff told me that they don't know how to do Post Martum and top of it They put
