I apologize for the delay for the delay in submitting my review concerning my two visits to Crystal Creek first in April 2019 and June 2021 for hip fractures. I want to begin by saying It is the only times I have had to be there and I feel the care I received was excellent! After my most recent hospitalization I was so exhausted when I arrived at Crystal Creek. I was so pleased when the Physical Therapist said, " I remember you," it was a wonderful welcome back to Crystal Creek. The next day two other physical therapist came into my room and asked if I remembered them - I did. All my caregivers

Stop! Don't go here! 3rd floor staff was awful and it only took me a day and night to have an attack from being so upset with these 100 percent African staff on the 3rd floor. I went back to the hospital and am now in a wonderful place called On Point. At Crystal. Creek there was 1 tech for the whole floor and she told mè that right away and left mè. I can't even type all that happened to me in a short time but I knew I had to get out of there.i was so upset and short of breath that I went back to the hosp thank God. Beautiful place but 3rd floor is HELL. Big communication problem too.

My mom was recently here at crystal creek. Before admission we called asking their visiting hours because we wanted to make sure we could go see my mom all day everyday. They told us visiting was from 9-9 (not only to my sister but to my dad as well, by 2 different people). The day she was admitted my dad was not allowed to see her because he “did not schedule an appointment” even though he spoke to the coordinator who said he would be able to see her. My father came to Crystal Creek for short term rehab after a terrible fall in his home. The nursing staff took great care of my father and the rehab team went above and beyond to encourage my father to participate. He ended up moving into the assisted living at Crystal Creek and we are equally as happy. The staff made the move easy and Dad felt right at home. He has a spacious apartment and even met a few friends that watch football with him. Thank you Crystal Creek.

After back surgery I came to Crystal Creek to use their rehabilitation facility and I can honestly say the occupational therapists and the physical therapists did a wonderful job with me. I came in with a lot of pain but David, Debbie and Akash worked with me daily. They encouraged me and then honest with me about doing all exercises. As I watched all the therapists I noticed that they really cared for their patients encouraging them to work hard. So thank you Crystal Creek and rehabilitation for your kindness in taking care of all your patients.

My grandmother lived in one of the most expensive communities that Dallas has to offer and we recently moved her to Crystal Creek to the Assisted Living due to lack of service at the prior community. Best decision we ever made! My grandmother is happy, loves the activities and friends she’s made and she said the food is great. Communication with nurses and caregivers has been great also. She loves her beautiful apartment that overlooks the wooded courtyard and she and her new neighbor love having coffee on the patio in the morning. My husband has received absolutely wonderful care for close to three years at Crystal Creek! We started in Rehab, moved to Assisted Living and currently are in Skilled Nursing. All the areas of care are exceptional! The nursing staff, the CNA's, the food services, the maintenance team, reception, housekeeping, therapists, the activities team, transportation, the insurance management, human resources, Attending Physicians and the Administration ALL work together to provide EXCELLENCE in Care, day and night!! I counted 35 "favorite" staff there! We highly recommend Crystal Creek! Their encouragement

My mother has received excellent care from the aides for 5 years. She has daily medical needs and lives in assisted living with extra care. This is a great place for someone who wants to live in assisted living with extra aides instead of moving to nursing care. Her apartment has plantation shutters and light plank flooring on the 2nd floor and looks out on giant pine trees in the back. So her view is great. Activities are well thought out to various museums, plays, movies, etc and the residents have input as to where they would like to go. The maintenance department also takes good care of any
