CAREPax will assign a care coordinator to manage all your medication needs from start to finish. Your coordinator will schedule a meeting with you and/or your loved one to explain our CAREPax program. Once you decide to participate, your coordinator will stay in contact with you and your healthcare providers on a regular basis to ensure your prescription needs are met and to make necessary changes as your medications needs change.
Once you join the CAREPax program your medications will be automatically delivered to your home on a monthly basis. Have a change to your medication? No worries. CAREPax will devliver your new prescription to your door and make the necessary changes to your CAREPax plan on the following month.
