Services
The Patient Portal is an online communication link between patients and their providers. Includes health promotion, disease prevention, health maintenance, counseling, patient education, diagnosis, and treatment of acute and chronic illnesses in a variety of health care. Primary care is performed and managed by a personal physician often collaborating with other health professionals, and utilizing consultation or referral as appropriate.
Process should not take more than 10-15 minutes. Please bring it completely filled out to the office or send it through fax to 281.768.7908. Please Do NOT e-mail this information. Our office follows by HIPAA regulation, therefore we do not ever release medical records from our patient without previous written authorization.

Dr. Jaime Duarte is the best! Very knowledgeable. Suffered with skin rash every summer in the past 5 years. Normally, it would last 3-5 weeks. 2weeks ago, got the rash again. Referred by a friend. If I could give this place a no star I would . They won’t let you in without a mask and reiterated several times before the appt that we must wear one which I’m fine with ,yet you come to the clinic and no employee is wearing it. This location is very unusual there is a receptionist but I’m not sure what her purpose is to be there since she directs you to the machine right in front of her which is where you talk to a person and the person asks you all your personal information such as name birthdate and address and insurance information for the whole lobby to hear anyone can just write down

I’ve been going for years. The drs do care for their patients. But some of the staff is rude there times they don’t even know what they are doing. There’s two staff workers there that been there for years they Wk in the front . If the other staff was like those two lady’s it would be so much better and would be a 5 Star ⭐️

Terrible experience. Very unprofessional, not polite, employees in street clothes. I know lots of doctors and med field colleagues, they would not be happy with this practice. I was giving them the benefit of a doubt and actually went to my 2nd appointment....but now, i guess i will do business elsewhere. I walked out, and she said "have a nice day" in a very uncivil manner. Terrible
