Peninsula HealthCare Connection (PHC) promotes integrated primary and mental healthcare and intensive case management for homeless individuals or those at risk of becoming homeless in Santa Clara County, regardless of their ability to pay. We will collaborate with local hospitals, healthcare providers, social service agencies, and similarly committed partner agencies to this end.

Services
The clinic is located within the Opportunity Center in Palo Alto located at 33 Encina Ave, Suite 106, Palo Alto CA 94301. To access the clinic, enter the Opportunity Center through the family side (left side when facing the building) and proceed down the hall. Free primary, preventative, and mental healthcare to homeless adults or low-income residents within Santa Clara County.
Award-winning social work case management program of Peninsula Healthcare Connection, founded in 2002. New Directions is a community-based case management program for individuals with complex medical and psychosocial needs, many of whom are chronically homeless.
Our intensive case management model and "whatever it takes" philosophy have proven to be effective in reducing the frequency of emergency room visits and hospital admissions, and providing overall improvement to quality of life for our clients.Referrals to the New Directions program come directly from partner agencies.

The vast majority of whom are adults experiencing or at-risk of homelessness residing in North Santa Clara County and South San Mateo County.
