After working here and seeing the care these residents received from the majority of the staff and the blind eye the management from unit managers Jocelyn to DON Amanda all they way to ED Brent/Drew and the NP Shannon aka Dr death as most of the staff call her, since she likes to over medicate and never actually put hands on patients to assess them. She acts like its more of an inconvenience to even be bothered with them. Through my time there, We had an ant infestation to the point they were crawling in patients trach, the nurses were told specifically not to document on it by the DON and ED.

Great staff and environment. They take great care of all the patients there. I have even received calls from the director of nursing with updates and addressing any concerns. When we were allowed to visit the facility was always clean and smelled nice. Now I visit at the window and they are always very accommodating to give my mom the phone and get her to the window so I can see her. They have recently gone through change in management and now have full time staff which creates consistency for my mom. I appreciate all they have done and are doing to allow visitation again. I feel like people only

Since coming to work for Opus I have found a company that truly cares for their staff and residents. The staff go out of their way to care for the residents. They hold the hands of those who have no family and make them apart of the their family and the Opus family. Is this facility perfect, no but thats because nobody is. One thing we strive to do is care and thats more important in my opinion then being perfect. We have a great nurse practitioner that comes 5 days a week and also follows up on the weekend and a wound PA that comes once a week.. I am so proud to say I am apart of this team.

My mother was at this facility from March-September 2020. Overall, it was a very pleasant experience for both my mother and I. The nursing staff kept my family well informed of my mother's condition. My mom said the rehabilitation staff was excellent. We both felt they did a great job at helping her regain strength so she was able to come home quickly. I would choose this facility for future needs if they occurred.

Although most of the nursing staff is great, there are a few I would have loved to slap. The cleaning staff were very nice and took a minute to talk and listen. PT & OT staff wonderful. They are the only reason I haven't lost my mind. Well them and God. Unfortunately like any nursing home it is under staffed which is why this place is so bad. I do use the term "Nursing" home because that's exactly what it turns out to be. If this was truly a rehabilitation place it would be a whole lot better. I was in a rehab in the past and it was a great place to get better. That's why I wanted to come here

Nicole Lawrence is an amazing nurse! She's bonded with my father, listens to his wants and needs and addresses them appropriately. I'm so thankful she's there to help take care of my daddy!!! The rest of the staff has also been great in taking care of him in the year he's been there! Thank y'all so much for doing what our family cannot πππ

Awesome ! My loved one was there and on two instances, we were about to leave the facility. Due to the food being horrible, and the CNAβs being very poor. One thing I most say they do have an excellent nurse name Jeanne who works on Rehab that tries to save the building. My mom was sick, and she figured out what was wrong in no time. My family felt good when she was working. Therefore, I would give the nursing staff four stars.......

I dropped by to spot check this facility a couple days after a nice meeting with straight forward and helpful admission manager. The place had attractive open day rooms which are actually the lobbies(neat). The place seemed busy with med techs and nurses checking on residents. There was a piano on each side of the facility and a family member playing some beautiful tunes. The place seemed lively. There was less of a feeling of a hospital setting and more a residential feeling to the place, which can be very attractive given your orientation.
I did NOT witness any foul odors and fear a lot of nursing
