Services
Dr. Perales is board certified Family Doctor and Clinical Assistant Professor of Family, Community and Preventive Medicine at the University of Arizona College of Medicine - Phoenix. Prior to opening Live Well Family Medicine in Chandler, AZ she worked as a primary care physician and urgent care physician for the Maricopa Integrated Health System in Phoenix, AZ and for Kaiser Permanente in northern California.
Dr. Zuzak answered all our questions promptly and professionally. My whole family of four went for the visit and she took the time to listen to all of our concerns without us feeling rushed. I've been to 7-8 different doctors in the last two year. Dr. Zuzak is the first doctor who actually took time with me and showed actual interest to improve my health.
Live Well Family Medicine has changed our electronic medical record and we are using a new Patient Portal called Elation Passport. I saw PA Haymee Lucio for my well-woman’s exam and my annual physical. This was my second time seeing her and I was very pleased with her bedside manner and the rest of the office staff from the from desk to the receptionists. She made a not-so-pleasant exam not all that bad and she seems very thorough and caring.

Dr Perales has been my dr for 6 years. I’m grateful she is in business and feel like she addresses whatever my concerns are. I generally don’t have to wait very long and when I’ve needed her to she has been able to squeeze me in. Her office staff have always been polite and extremely helpful. I would highly recommend her to anyone that is looking for a general practitioner.

I’ve been going to LWFM for a couple years now and I’ve always gotten great service from the doctors and the office staff. Elizabeth and the rest of the personnel have always been very welcoming and been helpful with any questions I may have. Always goes above and beyond to ensure I’m taken care of.

I scheduled an appointment with Dr. Hernandez for a CPE. I was so very impressed with his bedside manner, the time that he took to go over my medical history and his energetic personality. Also, the lab technician was so sweet and helpful. Receptionists was very professional and the medical assistant as well. I would definitely recommend Dr. Martinez and his office staff. Is a little hard to find a good pcp but I am so glad I scheduled an appointment.

Dr Donesa Zuzack has been life changing for me. A provider that listens, understands, and is solution oriented. She has helped me get my health back through motivation, accountability, and support. She’s helping me reach medication goals so that I can be the best version of me. I have recruited my husband to the practice who is someone who avoided doctors. I know that if my medical needs change, Dr. Z will be there to guide and support me.

Dr Perales is the most caring and intelligent Doctor I have been to in decades. She listens to and has empathy for her patients. I have had some really bad experiences with other health care providers and I am so grateful that I found her. She is always positive and thoughtful. Since coming to Live Well Family Medicine I have had nothing but amazing care from an amazing team! Front staff is very sweet and are always welcoming. The doctors and PA are some of the best I've met, they care for their patients and the relationship you build with them is incredible. They become like a part of your family! Absolutely amazing care and I am so glad I've brought my family here.

I started going to live well at the recommendation of my fiancee and have thought the staff is great,doctors are thorough. they spend a good amount of time with you and explain everything, answer all of your questions. I see Dr. Hernandez and had a great first impression right away. Made sure he had a thorough understanding of my history and helped me when I had a second degree burn. Taught me how to apply the cream if my fiance was working and couldn't. overall the office has a great vibe, great employees and awesome physicians. I recommend this office to my coworkers and friends/family.
