Impact Health Direct Primary Care
Direct Primary Care is a membership based model of primary care in which the physician directly contracts with the patient for their primary care. The patient pays a monthly membership fee for their family medicine care in our office. We forgo insurance payments which allows us to eliminate a lot of the administrative burden that currently weighs down traditional practices.

This allows us to restore the patient-physician relationship and provide personalized, transparent, and affordable care for our patients.
Services
Dr. Sjogren is a board certified family medicine physician in Ennis, Texas. She was born and raised in Texas. She graduated from the University of Texas at Austin in 2009 with a Bachelor of Science in Human Biology and UNTHSC Texas College of Osteopathic Medicine in 2013 with her Doctorate of Osteopathic Medicine.
Impact Health Direct Primary Care is a Family Medicine practice that provides services under the direct care model. We do not accept or bill insurance (except Medicare) and we don't charge our membership patients co-pays for their office visits. Our monthly fee covers most primary care services that we can perform in our office.
Direct Primary Care or "DPC" is a fast growing membership model of primary care that puts patients first. DPC practices contract directly with patients to provide primary care services in their office. We do not accept insurance, but rather charge a low monthly fee for membership. This allows us to provide affordable, personalized and transparent care for our patients.
Same or next business day visits for urgent matters. Because our patient panel size is smaller than a traditional practice, we are usually able to get our established patients in for same day or next business day visits when urgent matters arise. This means when you're sick you'll be receiving care from your doctor who knows you and your medical history.
Our micro-needling device infuses scientifically-formulated solutions into the skin simultaneously during the treatment to address a variety of skin care concerns. The best part is there is little to no downtime with this device. A maintenance program typically consists of micro-needling treatments every 4 weeks for about 6 months.
