Our mission is to be the facility of choice for patients, employees and physicians by providing high-quality care to our community. At Green Meadows Rehabilitation & Nursing Center, the pillar of our operation is the delivery of quality healthcare in the most appropriate, safe, patient-centered care environment.
Rapid Recovery is a specially designed clinical excellence program that is personalized to each resident we serve. The program integrates all levels of care from your hospital stay, rehabilitation at our facility, plans to return home, and communication back to your primary physician. Creating this continuum of care helps each patient return home quicker and helps prevent rehospitalization stays.
This level of care is for those needing a short rehabilitation or nursing stay in order to transition back to their prior level of care. We offer physical therapy, occupational therapy, speech therapy, and respiratory therapy up to 7 days a week. We also utilize our Rapid Recovery Program to personalize your stay and get you home sooner.
This level of care is for those needing a higher level of skilled care that can no longer manage at home or assisted living. We offer a full activities program, individualized plans of care, and nutritional services to meet the needs of these residents which includes a full range of high-quality health care service that is available 24 hours a day.
This level of care is for those seeking end of life care when treatment is no longer possible. This treatment is focused on emotional well-being, comfort, and pain management. Our facility is partnered with various Medicare Certified Hospice Programs and can also deliver facility directed Palliative Care.

I put a dear old family member here for rehab to strengthen his legs and stability after being in the hospital for “Failure To Thrive”. I was out of town and could not tour the facility first. Had I saw this place ahead of time, I wouldn’t leave my dogs there.
They put him in a lockdown unit because they were afraid he would “Elope”. This man is 91 years old and lived alone for 45 years and up until 3 weeks ago, drove everywhere. He did not get the therapy he was suppose to, his room was worse then a jail cell and dirty. Banged up, bruises on him and looked like he hadn’t even showered

A five star for your receptionist. She is both professional and cheerful to interact with. As for the rest of your staff they all get a zero. Ask a nurse for an update on your spouse and you get ignored. Ask an aide to refill the antibacterial soap and you get ignored. Leave a message for one person in a professional position on two occasions and you get no reply. Ask for something for the patient and they act like your are causing them a major hardship.
Don’t say that you are short staffed because of Covid! How many new patients have you admitted to Green Meadows since Covid started? I do say

My mom has been here just under 3 months and is doing better than I’ve seen her do in a long time.
She loves the social interaction and scheduled activities that are provided.
I’m amazed at how accessible the key staff personnel are as each of them has given us their direct numbers and are very responsive whether by phone or email.
So far, it has been a very positive experience which which was beyond my expectations.

Today the nursing supervisor personally worked with my dad, Chester Lakatos, regarding his clothes….which we can’t find.
What she said, and how, was excellent. Now we have to see the follow up.
But the supervisor nurse(who came in because the regular nurse supervisor called in sick) did an excellent job. I want to thank her for making my dad feel better…listened to…. Today.

Meadow Green has provided my mother with excellent care after she was in the hospital for over a month. Even though it was a short stay all the staff was pleasant and very helpful. I can’t thank everyone enough for nursing my mother. The accommodations are excellent and the room was very clean. If anyone needs nursing or rehab this is your place.

If I could I would’ve given this facility a negative star! “Green Meadows” came highly recommended… and we believed their promises, to take care of our loved one and that they were in the absolute best hands, care and facility! Outside this place is gorgeous and gives no inkling to the horrid room my family member was in! The visiting and front areas are also highly deceptive! I would’ve never known that this place would be hiding the “Michael Myers (HALLOWEEN) hospital scene” just a few feet away! No one is here to stay…and some of us will grow old, just don’t make the mistake

Our mother has been at Green Meadows a little over a year having recovered from COVID-19 and she receives very good care from many of the staff. It has been very comforting for us as family to know she is getting such good care and the staff has been wonderful to us as family. In particular Brittani the activities manager has been awesome and very caring.

Our father has been at Green Meadows throughout the entire COVID crisis to date, and we have been very impressed by the facility, from the very top of the house (administration) to the nursing staff, the social workers, the attending physicians and other care givers. The facility director has shown great leadership throughout what must be a very trying time, and has regularly kept family members informed about the most current developments at the facility, the safety measures in place, the other ongoing developments. The nursing staff has been very caring of the residents, and the social workers
