I am an orthopedic surgeon trained at Northwestern University in Chicago, Illinois that has been in the Pacific Northwest practicing with an excellent reputation for nearly a decade. I proceeded to work with multiple thought and opinion leaders for two years to learn additional expertise in shoulder surgery following completion of my residency.
Injuries to the AC joint are common and may be degenerative or traumatic. Most commonly as we age, bone spurs develop in the AC joint as a result of arthritis, and when it is symptomatic may produce a very typical pain for patients that is easy to diagnose via physical examination. Treatment may range from NSAID use to most commonly a cortisone injection.
Whether it be patch augmentation during rotator cuff repair used from a dermal donor (skin) from animals (most commonly pig), spun down platelet rich plasma (PRP), or bone marrow concentrate (BMC or BMAC) which contains precursors to stem cells the use of Biologics to supplement healing is a very attractive option to offer patients.
The shoulder (the humeral head - or ball - and glenoid - the cup) are surrounded by robust ligaments and capsules (you can almost think of it like a balloon) that provide stability to the shoulder and are normally, very elastic. These allow a huge range of motion in the shoulder that is unparalleled elsewhere in the body.
Normally shoulder anatomy delicately balances the humeral head into the cup of the shoulder (the glenoid) with boney and soft tissue stabilization. A good portion of this stability comes from the rotator cuff which centers the humeral head into the joint as it appears in this X-ray. The tendons of these muscles can tear however, and this suction effect can be lost resulting in increasing superior (or upwards) migration of the humeral head.

Dr. Schwartz is very skilled and good at what he does. I had a complete reverse shoulder replacement. Any questions that I had, he answered them and put me at ease with his kind bedside manner. He was amazing when my daughter broke her elbow in 2 places playing basketball her senior year. He was informative, spoke to you in terms you could understand explaining the procedure to repair her elbow, he was also kind and reassuring, and most of all, he related to you as a person. I had years of shoulder pain/discomfort. Spent more than a year of conflicting diagnosis; until......Dr. Swartz did a basic test, confirmed issues, and made it easy to understand.
