Services
At Oak Street Health, our primary care providers are specialized in caring for Medicare patients and seniors with a preventative care focus that strives to keep you healthy and out of the hospital. Our state-of-the-art facilities are set up to handle what seniors need most, including community rooms to offset isolation, in-center pharmacies in some centers, and even behavioral health specialists.
Since our founding in 2012, our mission has been to build a primary care delivery platform that directly addresses rising costs and poor outcomes, two of the most pressing challenges facing the United States healthcare industry. Older adults have specific healthcare needs. Many don't receive it. When they do, appointments are brief, in-network doctors can be far away and they don't get personalized treatment plans.
We believe everyone should have access to high-quality healthcare. That's why we treat seniors on Medicare for the kind of ailments and conditions older adults face. Our Medicare primary care doctors, and entire care team is specially trained in the support of seniors' health, with a focus on preventive care because it's far easier and less costly to treat wellness than it is to treat sickness.
Patients appreciate more time with doctors, getting rides to and from appointments, same-day scheduling and around the clock telehealth availability. From fitness classes to help understanding Medicare, we're here to give you more time, more support and more guidance for your health.
Our doctors specialize caring for anyone with Medicare, including Original Medicare Part B, select Medicare Advantage plans, Medicare Supplement or Medigap plans and Medicare-Medicaid Plans.Call (888) 812-1183 to find a Medicare doctor near you and learn more about becoming a patient at Oak Street Health.

Rebuilding healthcare is hard work. That's why we're looking for people who bring the best of themselves to everything they do. Because we're here to make an impact for our patients and the communities we serve. We care about our employees as much as our patients. That's why we offer our employees an excellent comprehensive, and flexible total rewards package.

When you walk in the atmosphere is similar to a restaurant...Welcome ...great to see you etc. The real medical service consists of ordering medications that were prescribed by other doctors any other issues you will be instructed to call 911. Afterward call Oak Street Health to let them know what happened.
Which breaks my heart because having worked with many hundreds of businesses over the last 40+ years it is the greatest business model for medical care I have seen, and that we in our society need.
But when you are a fully confirmed client and walk in on crutches with a full leg cast and they

I don’t think Oak Street Health follows their mission very well. They claim great healthcare but yet my sister does not get that. Every time you call the office it’s directed to a customer service department and they claim they will forward your message to the office. I have called them them numerous times (for months) and I have yet to receive a call back for my sister. She had a chest x-ray on September 28th and no one called with the results. They claim they don’t have them but I spoke directly with the radiology department at SCH where she had this done and they faxed it over on September

I have consistently been unable to contact the office since before Covid-19. When I did get through and leave a message, I would never receive a call back. Most times I would get a recording and not get through to leave a message. The psychiatrist I had initially, apparently left the company and I was not informed. At the time, I was in crisis. Asked to be contacted. I wasn’t contacted. After a week, I contacted my primary and explained I was in crisis and that Dr. O did not contact me. My primary stated, “Ok, thanks for letting me know. I will send a message.” Really!?! I heard nothing for

I read so many one stars. I am so sorry you are experiencing this. It might be the location. I am enrolled with 43rd & State Street. This location for me is awesome.
From my primary physican Dr. Shareeka Edwards, technicians, reception desk and drivers have a personal professional approach. I feel taken care of. They follow up from referrals and review test results with me. During the beginning of the Pandemic I received WEEKLY calls to see if I needed anything. Maybe the other locations should seek guidance from 43rd Street.

At this point...I'm astounded as a retired/disabled RN how patients (me), have unbelievable lack of concern for the patient. This is the worst care I have received. My phone appointment was ignored last week. Now finally have another phone appointment and didn't call. No continue of care or coordination with other physicians. LUDICROUS!!!

I don't know how they continue to operate. I have had MULTIPLE situations where I have called for my mother and they have not returned my call. When I've complained they return my call at 4 pm. Since the call went right into voicemail, I called back a minute later and now they are closed. Urgent questions are of no concern to them in terms of returning calls. There is no effective communication between patients, caretakers, office staff, and doctors. The only good thing is that they offer transportation, beyond that, this is not the type of place for my loved ones. My mother has

I am not a patient, never have been and now never will be. My insurance tried to switch me without permission and without having heard of Oak Street. I immediately fixed that, but have been being harassed by constant phone calls and mail from Oak Street ever since. This has been going on for months! No matter how many times I call to get it stopped, they say they'll fix it and don't! Then, I tried to find other ways to contact them like email or a help page, and they have nothing at all like that...phone only. That makes no sense when most of their business seems to be people on Medicare, and therefore

This is by far the most unorganized Medical Center. My senior citizen parents go to his center. I am trying to move them. I like the concept of the health center however it is not as run smoothly. My biggest frustrations are referrals. The wait time has been at times up to 2 months. When I address this concern with the nurse practitioner I was told the referrals are done by a third party and unfortunately they do not have a time frame. However it should not be taking 2 months. She stated she would put on the order that the referral was needed as soon as possible. However when with the referral

I'm quite satisfied with the care I receive at Oak Street Health on Devon Avenue. My primary care person is physician's assistant, Masha Chisick. She is very thorough and thoughtful. All the practitioners there are women. A bonus is that they pick me up and take me home in a van. I have rarely had to wait to be seen if I have an appointment. They have an on staff insurance consultant who helps straighten out insurance matters and gives good advice. I was skeptical at first but since I am getting such good care, I'm very happy with this place.

Update: Was told to take my mother for a Ecco for her heart...and was told to call back a few days later for results... I have called 3 times and here we are a month later NO CALL BACK. I called the office and was given the run around on the phone by someone who sounded as if they were under the influence of something. The doctor would not get on the phone. Was told the nurse was on vacation and was told the person in charge was on vacation...I was then told I would have to come pick up the test results and wait for a nurse to come to my moms house on the 31st to go over the results! REALLY? Mind

I have at Oak Street Health a few years now, and have had good experience with the doctor. This past April my doctor pasted away, and I was scheduled to see the NP today at 8:40 am. I get to the clinic , and the receptionist said she could not find me in the system. Then when I did not respond at all to her just gave her the look "like what do you mean", she said arrogantly that they get new patients all the time and that is why she asked me that question. She did not know how to intake, me then she claimed my appointment had been changed, and I was left a voice message. When I ask who

Today was my mother's 2nd appointment at Oak Street Health in Oak Park. I see a lot of bad reviews however it has not been my experience with my mother. They pick her up and drop her off and no wait time in either situation. Our first appointment, we waited a long time but when it was my mother's turn, the doctor took her time with her. I could see why they take a long time- maybe not schedule so many appointments (but at the same time, I realize they have to as some people cancel or no show appointments - this is a business and they all have a bottom line). The appointment today
