Form doctors, nurse practitioners, physician assistants, and medical assistants will focus on the health needs of our patients and assist in the prevention, diagnosis, and treatment of illnesses and diseases. Our doctors look at all areas of the patient's physical and mental health, as well as any underlying chronic illness or reoccurring illnesses.

I waited 3 hours for a covid test. No one said nothing to me; I had to get up a few times to see why the wait was so long. I've noticed people come in after me were seen within 20 minutes which of course would make any person upset and confused. The staff only told me they weren't near my number yet even though people went and families groups of 5 went before me. I Will never comeback. No one deserves that treatment.

No matter I was for 2 hours under the rain, cold weather, 40 degrees, and my toes were frozen because I was in line outside I have to say this is the only place in 10 miles to have your results for covid19 in less than 24 hours. Other places need appointments and they schedule for some days in advance.
This place can be a some better organize but they do all they can with the 111 people in the 2 hours I was there (I was 111 number).
The test was fast; after I was inside and filled the simple easy form, I was test in less than 18 minutes. To be a Saturday, 10am in the middle of omicrom covid19

This is a group of caring and loving servants, who work tirelessly to provide their patients with the best possible experience.

I went for a Covid test the week of Christmas which was an extremely busy time. I was so impressed with the staff’s process and flow of managing so many people coupled with their terrific attitudes. They were courteous, friendly and conscientious, and the best bonus was results in 24-36 hours! I would highly recommend Forme Medical Center in White Plains.

Aside from the "waiting on line", time, (which is understandable due to the circumstances) the service was exceptional. The employees were very friendly and helpful and extremely efficient. They made you feel comfortable and were very informative and attentive. I am very thankful and satisfied with the services received.

Amazing staff!! Super friendly and attentive, Yes the wait for a COVID test before the holidays was long, but they were moving at such a stead and fast pace once inside. Super efficient when it came to intake forms and insurance, once inside the wait was less than 10 mins In and Out. This will for sure become my new go to for Urgent care.
