Our experienced clinical team creates custom plans of care for each of our residents in order to best improve strength, mobility, and independence. Our goal is to help our residents recover from surgery, serious injury, or illness, and regain the necessary skills to return home. Our large, state-of-the-art therapy room provides a comfortable and private setting, and therapy is provided seven days a week.

I read all of the reviews everywhere. They are mostly bad. I read the reviews because a friend of mine was sent there. Such a bad facility that she is leaving. Your staff and nurses leave a lot to be desired. Especially the staff who chided her when she asked why everyone got breakfast BUT her. That is part of THEIR job! You should be inspected by the state and/or closed.

Physical therapy was out standing nurses were attentive to your medical needs and the food was great they helped me to a speedy recovery

Not good keeping family informed .When u call just rings. They don't give there name when they pick up phone. Not a very good way to keep up to date on your loveones.

My father in law was there for about 8wks. He was in a lot of pain and was suppose to get pain meds every 4 hrs....they were constantly 2 hrs late with his meds. He was suppose to have a no salt diet....they don't accommodate dietary needs.

This is one of the best places I have been to receive physical therapy for my leg problems. The therapy staff is extremely knowledgeable and helpful. The nurses and staff are genuinely caring. I am so happy to haven been referred here for therapy. I am now back to my normal daily life. Thank you for all the support!

My friends mother is here thankfully not for long it’s a disgusting place. Most of the staff is useless and doesn’t care i witness a woman in need of a bed pan and the staff was no where to be found. The place is dirty and old and not safe. My friends mom had a gross smelly mattress with a hole in it that was leaking a fluid that looked like blood. There’s a male nurse that took care of it and agreed about out our complaints and got her a better mattress. I wouldn’t let a dog stay here do yourself a favor stay away from this dump

Unacceptable! Came to visit my grandfather on Father's Day with my parents and my mother had to notify the staff that on our way here we noticed a resident wheeling himself down Thornton St wearing only a hospital gown. Totally unacceptable! Today was Father's Day and you would think they would have extra staff given how many people come to visit their fathers and grandfathers on Father's Day... But no, I counted four aides for the entire building! One consider even had to take her mother to the bathroom herself! We will be working on getting my grandfather out of here and into a nice place that

I would never put my mother there! Friends of ours had their mother in that facility, and I went with them to see her. That facility appears nice but after spending few hours with my friend and her mother, I was absolutely certain that if- God forbid- my mother ever needs nursing care I would quit my job and take care of her before I put her to place like this! We could hear the staff complaining about not having enough clean linnen & towels to do their jobs. Call bells went unanswered for upwards of 20 minutes! There was only 1 nurse and 2 aides for an entire unit of almost 50 people! Even the

THEY DO NOT HAVE CENTRAL AIR! ! The hottest days we've had this year. 98 degrees today. It's not safe given the age of the population in their care! These people are at the most highest risk in this kind of weather. It's hotter than hell in here!!! There are little window air conditioners in the patients rooms. However they're turned up so high to get air out to the common areas that the residents either freeze in their room or sweat in the hall.

I was in the rehab part after having my foot run over by a school bus. I really enjoyed staying there and the staff was great. I cried after leaving the center because everyone became a family to me.
