Services
At Central Park Rehabilitation and Nursing Center, our singular goal is to provide the best quality care for our residents and peace of mind for their families. An individualized plan of care is developed in partnership with the resident, the resident's family and the entire medical staff. Every effort is made to incorporate the resident's preferences and routines into the day-to-day treatment.
Central Park recognizes that family members are the major care givers for disabled adults, elderly parents, or chronically ill adults. Providing 24-hour care is a difficult task while attending to the needs of your loved one while trying to retain a job outside of the home and the usual daily activities.
The Bridge Program is designed for the participants who have recently been admitted to the rehabilitation/transitional care unit and who have reach a certain level of independence, but require additional monitoring and supervision of care. This program allows for additional time at the participant's pace all while living at home and receiving the additional support such as, medication administration, wound care or personal care services.
At Central Park we understand how challenging it is to care for your loved one on a 24-hour a day basis. A number of our participant's caregivers' work full time jobs outside the home. We have listened to stories from caregivers feeling guilty leaving loved ones home for extended periods of time unattended, in order to keep their employment.
Being diagnosed with a serious illness is a scary time for a resident and the resident's family. At Central Park our staff works with the resident and the resident's family to provide comfort and achieve a high level of quality of life and dignity during the dying process.
It is important to offer the ability for the resident and resident's family to experience a supportive environment and allow family members the opportunity to share meaningful time with their loved ones as we provide for the physical care giving.An additional offering is a partnership with Hospice of Central New York to provide end-of-life care within the nursing home setting.

I WOULD GIVE ZERO STARS IF I COULD. Please do not send your loved ones here! My grandmother has been severely neglected, and showed up to a doctor appointment this morning covered in food, urine, and filth. The doctors were stunned!! They said she has not been bathed or properly cared for in a very long time. We are taking matters up with a social worker and hopefully getting her out of this disgusting place. Also, my grandmothers belongings have consistently been stolen from the so called “caregivers.” I could go on for days, but please do not even consider this place for a minute!!

If you are having trouble contacting the facility and/or having them take any action, please search for the support group on Facebook to get in contact with other families facing the same problem.
Separately, I would recommend contacting the NY Long Term Care Ombudsman Program (LTCOP) for Onondaga and speaking with the coordinator.

My father went in to Central Park after a hospital stay in March 2021. After rehab, the rehab facility sets up aftercare with a nurse and/or therapy. Central Park called Kindred At Home, and Central Park was told that they were not able to take on my father as a client. My father was released back to his independent living facility by Central Park. Since then, 9 days later, there has been no contact with Central Park for help with a nurse or therapy. I"ve called, my sister has called, his insurance company has called Central Park. There has been no response from Central Park. My father is 86 years

Do not send family members to this nursing home if you hope to see them again. The covid virus is rampant in this place and every week someone becomes infected. This means that your loved one must stay at least 14 days before the can be released! If someone else is found to be positive in the 14 days, the clock goes back to zero, they are held for another 14 days. Over and over and over for the last year, residents who are well enough to go home have been held prisoner without any way of going back leaveing !!

I was not impressed with the staff. The receptionist was on her cell phone when we arrived and when we left 2 hours later. When my father passed no one could locate his clothes. There was not a procedure in place. I was sent from the receptionist to the nurse to the custodian, activity director, then social worker. I was not satisfied or pleased with the staff and care received at Central Park.

My mother was at Central Park for nearly 4 years. She passed away at the end of 2019. The staff at Central Park became her second family. When I was not able to visit, I was not worried as I felt that she was always well taken care of. The facility even reached out to me recently regarding an upcoming planned memorial service for those that passed away and she was being remembered. I was completely taken back and it once again reminded me of the many thanks I have for all those that cared for her and treated her so great during the final years of her life. I miss seeing the many familiar faces

I'm glad I found this place. I can't say enough about Central Park Rehab and Nursing.
Upon learning that my mom would not be able to return home with me, the search began. I visited the three facilities that accepted my mom, and the difference between the two larger nursing homes and the smaller Central Park could not be more clear.
First, Central Park had the better Medicare rating.. but it didn't stop there. The initial tour by Kevin at Central Park was genuine and sincere. He listened attentively and paid attention to our questions. Central Park is a smaller, four-floor facility, and everyone

As a past employee of Central Park I cannot say enough good things about this facility. There is a real sense of community when you step through the door and there are a lot of staff members that have been there for numerous years and are extremely dedicated to the residents and treat them with the utmost respect! I would not hesitate for a minute to put my loved one here if that difficult decision ever came about.

The administrator and the nursing staff was so involved in my stepfather's care and showed such a great deal of compassion. I know nursing homes get bad reputations, but Central Park and the staff went above and beyond what was expected. The administration created a comforting environment. When my stepfather passed away they were there to support my family and offered the greatest amount of respect. I would definitely recommend Central Park to anyone in the Syracuse area! My loved one has lived at Central Park for many years now. Central Park has greatly improved over the years and I feel that they truly provide quality care and compassion for all of their patients. My family member loves the food and the staff that cares for her. I would recommend this place for anyone needing a nursing home in the Syracuse area!
