Medford Adult Day Health Center (ADHC) is a 6-hour day program designed to provide care and companionship for seniors and vulnerable adults who need assistance and supervision while maintaining their independence.

Medford ADHC offers relief to family members and caregivers, while reassuring that our participants are well cared for in a safe and nurturing environment. Our professional interdisciplinary team created a customized program that ensures our members from different ethnic and lifestyle backgrounds feel at home.
Services
Medford Adult Day Health Center (ADHC) serves communities in Medford, Malden, Somerville, and Cambridge. Our program is designed for older adults, adults living with dementia, and adults living with disabilities to receive individualized therapeutic, social, and health services for 6 hours a day in a professional care setting.
Medford Adult Day Care program is designed to meet the needs of functionally; cognitively impaired and/or isolated adults and seniors. Our structured programs provide a variety of social, physical and medically related services and interventions, in a protective and stimulating setting. We coordinate the participants' individualized plan of care with families, caregivers, their primary care physician and other involved support agencies.
Our multidisciplinary staff brings compassion, years of training, and commitment to meet the physical, emotional, and social needs of participants and family caregivers. Our multi-lingual staff members are certified in CPR, First Aid, and Alzheimer's Care. Medford Adult Day Health Center maintains registered nurses, and they maintain communication with primary care physicians and their assistants about client's personal needs.
Coordinated care is a priority when it comes to the welfare of individuals attending Medford ADHC adult program. Medford ADHC social worker is part of the interdisciplinary care plan team, who ensures participants needs and concerns are reflected in the individualized care plan.

The social worker meets with program members and their families to understand their needs, make the referrals to the community agencies and offer guidance how to navigate the complicated modern health and benefits system.Medford ADHC also provides case management. The social worker is readily available to families to assist with the challenges and worries of being a caregiver to our members.
Requirements: an active Massachusetts license; ability to speak and write fluently in both English or Mandarin; knowledge of care coordination, community service and delivery systems for the aging population. Bilingual Mandarin/Cantonese speaking is a must; the schedule will include weekends. Requirements: the ability to lift 50 lbs, assist clients with activities of daily living, experience working with older adults, CPR certification.
