Founded in Oakland, California, the mission of Roots Community Health Center is to uplift those impacted by systemic inequities and poverty. We accomplish this through medical and behavioral health care, health navigation, workforce enterprises, housing, outreach, and advocacy. Founded in 2008, Roots Community Health Center is a 501(c)(3) non-profit organization dedicated to improving the status of health of East Bay residents.
Roots implements its mission by providing high-quality, comprehensive and culturally appropriate health care, by implementing direct service models to increase access to care and thereby prevent the need for emergency services, and by coordinating cross-agency liaisons to ensure a more efficient, continuum of care in East Oakland.

Roots can be your medical home. As a Roots patient, you receive wraparound support so that you can make the most of our high quality, comprehensive care. Call 510-777-1177 to make an appointment, or schedule your appointment online. We work with you to ensure cost-effective care via insurance or sliding scale fees.
We depend on supporters like you to sustain our services and programs. Your contribution goes directly to the work we do to provide thousands of low-income community members a medical home, job training and other forms of support that break the cycle of poverty. Oakland STOMP is Roots' street medicine program that brings care to the people in homeless encampments. We deliver outreach, benefits enrollment, linkage to services and treatment, and medical care to homeless individuals throughout the city of Oakland.
