Services
Appointments: Office hours are by appointment. Please call in advance to schedule. If cancellation or rescheduling is necessary please give us 24 hours notice, as there are often other patients who would desire to take an available appointment time. The Consultation: At the time of the initial office visit, we will need a little extra time to become familiar with your medical status.
Forms: (We hate forms too): However, like all offices, some are required, including by the U.S. Government (HIPAA form - verifying how you want us to handle your private medical information). We ask that all new patients arrive a minimum of 10 minutes prior to their scheduled appointment in order to complete the registration process.
