If you are a current patient please use the online form to schedule your next appointment online. If cancelling an appointment please call at least 24 hours in advance to allow us to book this time slot for someone else. All medication refills require a 72 hours to have your prescriptions send to the pharmacy.

Don’t recommend! Their nurse practitioners first not a doctor, second of all the practitioners should not be giving out personal phone numbers to their clients and dealing with medication refills via text message! Like I was, this needs to be reported. Also don’t take somebody off your medication that you’ve been on already for seven or eight years they’ve done this to myself into many others. This is an addiction doctor not a primary care physician I do not recommend them nor will you get your prescriptions refilled Due to the fact they don’t respond to their fax machine request for
