The Family Health Centers of Southern Indiana strive to follow a patient centered medical home model. This means we want to be accountable for the majority of each patient's physical and mental health care needs, such as prevention and wellness, acute and chronic care.
We strive to provide health care that is relationship based on focused on the whole person, all while coordinating care across all elements of the broader health care system; including specialty care, hospitals, home health care, and community services.

You may stop by any of the sites in order to complete a registration packet. You can also click the forms tab above to access and print documents at home. You may request assistance from the clinic staff to complete the information. You will be asked for documentation to determine your income, insurance status and county of residence.
Over the years, this location has grown and is now funded as a Federally Qualified Health Center through the Bureau of Primary Care. The Clark site serves as the main location for services and has management agreements with the Floyd and Harrison sites. The Clark location is also a state funded Community Health Center.

At first, I thought this was a great place. As it turned out while unemployed, it cost me more to get two teeth pulled than it would have, if I'd went to a regular dentist. This place is for poverty patience. "Based on income". Dentists are becoming more like fly by night doctors as time goes. I need dentures badly, embarrassing as it is, because of my severe vitamin D deficiency. As I said before, they pulled two teeth that were bothering me on the first appointment. THEY cancelled the second, and would not pull any more when I had a second appointment, but charged me the same amount for the same

This place is TERRIBLE! I've been trying to get an actual human on the phone for over 2 months. I've been off of my antidepressant for a week now because of this place, going through literal withdrawal, and they still won't fill my prescription or call me back. I hate this place, these people don't care about anyone or their mental state.

A few years ago I had a dental issue and fortunately I found Dr. Erika White, DMD at the Family Medical dental clinic. She is amazing, as well as her staff, they truly care about their patients. I have had better dental care in her office than some of the high priced dentists in the area. Thank you and your staff for taking such good care of me and my family.

When I didn’t have health insurance My medical provider was Clark County Family Health Center. I received excellent care. I was blessed to have them because without them I would of had no medical care. I still see the dental department there. The dental department is excellent!! Thank you Clark County Family Health Center for all you do.👍❤️

If i could give zero stars i would. The only person in this office able to get tasks handled is the office manager. I have waited over a month for lab results and they still don't have them. I went in for a wellness exam and they couldn't file it correctly so my insurance is having a field day with it. The EXPECT you to know whats wrong and how to treat it when you come in and offer no help other than on cheep prescriptions, unless they are life threatening medications (then there is no help for you).And you have to love their ability (not) to answer the telephone. I am 64 years old and I went to the Family Health Center in Clark County at 1319 Duncan Avenue around five times. The only reason I went there was for high blood pressure.
I got a Medicare Summery in the mail and seen where I was charged for Smoking Cessation Counseling by this health center. I never had any counseling there to try to stop smoking.
The doctor I seen there ask me if I smoked, but that was it. No counseling. I didn't know doctors can charge for asking you a question. This was just a question the doctor there ask me out of the blue among other questions.
I called Medicare and reported
