Carolina Family Practice & Sports Medicine is part of the Private Diagnostic Clinic, PLLC (PDC), the physician practice of Duke Health. We are a 14-provider medical practice, now with three triangle locations. All providers follow the the quality and safety guidelines set forth by the PDC and Duke Health.
Dr. Josh Bloom attended Tulane University School of Medicine where he was a member of the Alpha Omega Alpha Medical Honor society. While at Tulane, Dr. Bloom also completed a Master's Degree in Public Health. Dr. Blount grew up in Edenton, NC, a beautiful little historic town on the Albemarle Sound in Eastern North Carolina.
Carolina Family Practice & Sports Medicine is located at 3700 NW Cary Parkway, Suite 110, Cary, NC 27513, halfway between High House Road and Route 54 (Chapel Hill Road). Take Kildaire Farm Road to Cary Parkway (head away from Route 1 and towards Preston Corners). After you cross over Route 54 (Chapel Hill Road), our office is located on the right after one mile.
As CFPSM patients, you now have access to Duke MyChart, a secure portal to request appointments and communicate with your healthcare team. When prompted for your activation code, please refer to the after visit summary you received at your last appointment. If you have not had an office visit and wish to set up your MyChart account, please call 919-620-4555.

The Athletic Trainer collaborates with physicians to optimize activity and participation of patients and clients by specializing in the prevention, diagnosis, and intervention of emergency, acute, and chronic medical conditions involving impairment, functional limitations, and disabilities.Athletic training is not the same profession as personal training.
