Western Horizon Hospice and Palliative Care, Inc. is a hospice care provider in Lake Forest, California. Our hospice programs are available to patients in the nearby cities and counties. Hospice is not about aggressive treatment but rather it is enhancing the quality of life for the individual during his or her remaining days.
At Western Horizon Hospice and Palliative Care, Inc. we organize regular visits by hospice nurses to you at home. They are trained in pain and symptom management specifically for debilitating illnesses and life-limiting diagnoses. In addition to nursing care, we also have hospice aides who assist with ADL and provide personal care.
Our Mission: At Western Horizon Hospice, we aim to bring compassionate care and comfort to patients with terminal illnesses. I have nothing but wonderful things to say about Western Horizon Hospice and Palliative Care. I have been challenged with looking after my mother for the past 15 years as she has struggled with debilitating dementia. She is now 95 and is completely incapacitated.
