Services
All nurses, helpers and case workers are polite, on time, and care. Physical Therapists Rue and Mitch are precious assets to be appreciated BIG TIME. - Martha T. I was a resident from 8/16 through 9/7. I would like to express my appreciation to the staff that helped me enormously with my recovery. I had the best Occupational Therapist Rue and Physical Therapist Sheldon; their hard work enabled me to bounce back and overcome a lot of obstacles.

My mother was billed $8,121.10 copay for her short stay at this location. This was what Medicare did not pay. Still confused by this huge fee. She stayed here half of January and part of February. My mother passed at the beginning of February. I kept receiving invoices for this amount from Erika Agapay the "Administrator". I've called over 10 times for this lady. She is either out for the day, on break/lunch or simply "unavailable". I have left several messages with no return calls. I informed this facility my mother had passed and asked what I can do regarding her copay

After my mother passed, I informed the center. Never once have I heard from them. I have called 9 times to discuss balance with Erika Agapay and she was never available, she was either off for the day, on lunch and/or break. So I left numerous messages to discuss invoice and to this day, 5 months later she has yet to return any of my calls. I even explained to the "receptionist" my situation and asked to speak to anyone that could help. She explained Erika is the only one that I can speak with. Odd, but ok. Very disappointed and sad by the lack of sympathy for their patient.

The San Pablo healthcare Center should do a better job of patrolling the perimeter of the facility. I noticed several homeless people with tents right outside the door. This is completely unacceptable and the problem needs to be addressed ASAP. If this situation isn’t corrected I will be Notify the department of public health.

I just went with my Husband to visit a client of his at San Pablo Healthcare center. There were about 5-6 glamorous people at the glamorous front station (I was asked to use the word Glamorous in this review) when we came in. My Husband and I came in and he signed in and we went on to visit his client, however, the ladies at the front could not get over it. They all turned to stare and started asking each other who "he" was there to see. THEY WOULD NOT STOP giving us dirty looks all the way down the hall. My Husband answered the question even though they never addressed him like a person. He gave

During these unprecedented times having a loved one in a skilled nursing facility can be incredibly challenging (as if it wasn't already a struggle on its own). Luckily, the staff at San Pablo Healthcare Center were resourceful and thoughtful enough to provide video chat sessions as we are unable to visit in person at this time. Relying on others to provide the care your loved one needs and deserves is incredibly difficult but the staff here certainly does their best and it shows.

Unable to visit due to county restrictions at this time but this facility is taking precautions of checking their staff's temperatures, not allowing any unnecessary personnel in the building, taking extra sanitizing steps all to protect their patients' well being during the pandemic while offering video chatting since visitations have been temporarily suspended for the time being.

I would never recommend this place to anyone. My husband went for recovery after hip surgery. He became ill and he was not taken care of. They were negligent and due to it all died. He kept telling me how cold he was, I mentioned it to the staff. His own Physician could not check on him as they have their own Doctor. so it was difficult to get help from the outside. He never should have gone there. Also, you could not get answers out of them. Was extremely frustrated with their front desk as well. No help what-so-ever.

I want too express my views about San Pablo Wellness Health Care Wellness Center.
I was a Resident there for 3 yrs 8 mos. So I'm going from each department and then conclude my final words.
Administration Intake:
I checked in around 6:00 pm. When they gernied me in I felt strange looking at residents staring at me, like a new victim. They did the first part of my intake then they gave me a peanut butter and jelly sandwich since the kitchen was closed for the night. They took me to the room I was assigned. I got on my bed and the first 2 months I was there I hibernated. Then a resident

I've been here everyday since my mother was admitted in September, so I think I know how it "flows". The facility is nice and the staff even better!! Can't say how highly I appreciate all the staff there specially Liz that has helped me set up transport and any little doubt I have when it comes to my mothers care there, she is very helpful and great!! . The facility has a funky smell but hey what can you expect it's older people and people with special needs there so.... but it's taken care of fast! The food surprisingly is good, with my mother being so picky she likes

I visit this facility weekly. What stands out to me about San Pablo Healthcare and Wellness is how welcoming their staff are. I am always greeted and feel welcomed, whether it's from the social worker, admissions director, front desk, maintenance director, nurse or even the residents. Keep making people smile, it makes a difference!

I've spoken to several family members of residents of this facility- all very happy with the care their family member receives. They aren't perfect but they try their best to accommodate what each individual wants and needs. Talking to family members who have experienced other facilities say that the facility is clean, staff is friendly and environment is very welcoming.

The stay was wonderful. They treated me right. Social services, rehab and charge nurse took care of me and handled their business. Joan in physical therapy made sure I reached my goal. The CNAs for room 22b are warm hearted , caring, considerate, magnificent, very patient and very professional. They did their job. I'm going home happy and content .

With its' rustic-western decor, this facility is immaculate and stylish. The halls were spotless and the nursing staff just amazing. This warm and welcoming place nurtures all the residents, whether they are short or long term patients. I was enthralled with the sensitivity and compassion of everyone working there. I noticed that there were many people there who were not "seniors". They offer rehab services for those who need intense physical and occupational therapy. Very special staff.

I would not recommend this to anyone. Deeply disappointed. They do not take good care of he patients. The person that went there DIED. They lie a lot as well. Also, do not return your phone calls. They also steal the patient's belongings.
The person that was in their facility should have been transferred to EMERGENCY IN A HOSPITAL. They failed to do that. The patient landed up with a heart attack and stroke. Was too late when THE PERSON was finally transferred. The PATIENT DIED!!
Their service was of extremely poor quality!! You can not proper answers out of them either.

My father was a resident at this facility for several months, and I absolutely would not recommend this facility to anyone.
Care: Most of the CNAs at this facility were nice people. However, there were a few bad apples, and I would never want anyone to be subject to them. In fact, one of the other residents told me, "Watch out for (name protected), she's nice to your face, but she's mean."
