Riverside Nursing & Rehabilitation Center is a leading provider of long-term skilled nursing care and short-term rehabilitation solutions. Our 91 bed center offers a full continuum of services and care focused around each individual in today's ever-changing health care environment. Riverside Nursing & Rehabilitation Center has been serving Lewis-Thurston County since 1970.

We are located along the Skookumchuck River within two miles of Providence Centralia Hospital and across the street from a church and a park. Our center is Medicare and Medicaid certified. Upon my arrival at Riverside, I was totally paralyzed form the waist down, and severely restricted in the upper body.

We understand you have many choices when selecting a health center for you or your loved one. A registered nurse, our Care Transition Coordinator helps patients navigate the Next Step program by serving as the point of contact between the patient, their family and all healthcare providers.

Throughout a patient's transition from acute care to post-acute care, and then to their home, our Care Transition Coordinator offers education, assistance and follow-up to help ensure a successful discharge with any necessary resources a patient may need to continue their path to recovery.

I recently had the pleasure of delivering Christmas presents for the residents at Riverside and I was very impressed with the level of care the residents were receiving. Each and every nurse/CNA there actually cared allot about each resident they were assigned. They made the atmosphere very happy and truly home like for the residents. Everything was clean and nice and all of the residents seemed happy.

Rating this as a prior employee, as with many places in Healthcare. It's understaffed. So much so that members are unable to provide the level of care that patients and residents deserve. I only worked weekends and many times I'd find my initials on the dressing I was changing.
Sadly, that was to be expected.
The reason for my rating is that many staff members were fired for refusing to work after receiving a positive covid test.
I didnt even receive a phone call or notification, just went to the scheduling app to find I no longer had access. That was not even a week after my positive test.

My dad is in this facility and not getting the care he needs. I have tried calling the place and leaving messages for them to call me back and not one person would return my phone calls. I you can't eat certain foods due to dietary restrictions you have to say you are allergic to that food .

I am so very grateful to the staff for taking such great care of my beautiful mommy!! She absolutely loves being there! She is surrounded by amazing staff! She calls them her family and they think of her as family as well. They always call to inform me of anything that is going on! Thank you for taking great care of my beautiful mommy

If your residents are under lock down then don't let the get delivery and waste someone's time. Answer your phones. Answer your door bells. Don't waste someone's time then have attitude about it. Rude workers, I refuse to deliver here again.

It is a common occurrence for this place to be severly understaffed for the amount of residents it carries. The RN and CNA do the best they can but unfortunately it is run by a corporation that exists elsewhere and doesnt really care what really goes on as long as the $$ coming thier way is at acceptable levels. More residents + less employees + low to nonexistent maintenance = profit. If people didn't cheap out and use facilities like these, they wouldn't exist. Otherwise be a better consumer. Some things cost less then others for a reason.

Mold and fungus in shower rooms.they want you to pay for skilled nursing once rehab is completed. Ther are non working exhaust fans in shower rooms,and some patient rooms. Non working heaters in some patient rooms. Overall the place is a dymp

My grandmother was her recovering from an issue with her congestive heart failure. While she was in their care I came to visit and found her in bed with her call light on crying and saying she had been waiting for help for a while. She had unfortunately had a bowel movement and was sitting in her feces. After 15 minutes of waiting I found a staff member to help her to the bathroom and get her all cleaned up. I pointed out old feces on the side of the waste can in her room and was told that was housekeeping’s job but that she would let them know. After she left the room I saw that she had left

"I came to Riverside straight from Harborview Hospital where I had undergone multiple intensive surgeries that saved my life more than once. I rolled through the doors on a stretcher. I couldn't even roll over on my own. The staff Riverside immediately went to making me feel comfortable. Being the youngest person at Riverside came with some interpersonal challenges and yet, the aides and therapy staff made me feel right at home. They joked with me, talked with me, showed me pictures of their own accidents, and gave me things to do to help keep me busy.
The therapy staff were always one step ahead

I spent a week or so Riverside after being discharged from Summit Pacific Medical Center in Elma, WA. It was run down and staffed with untrained nurses and certified nursing assistants. I left against medical advice.
