Brookfield Healthcare Center is a modern, state-of-the-art skilled nursing facility, located just north of the 5 and 605 Freeway interchange, on Telegraph Road in Downey, CA. We feature 70 beds and beautiful spacious rooms in either private or semi-private settings. Guests will enjoy well-appointed accommodations, a diverse activities program, nutritious satisfying menus, housekeeping and laundry services, and beautifully landscaped grounds.

Best facility , staff truly cares about their residents. They treat their residents as if they were family. Dietary department works very hard to prepare good meals and provide special meal needs and beverages. House keeping keeps brookfield spotless and the bedrooms clean. They also do the laundry. Nurses stay on top of all the medication and health needs. Social worker and business office are always there to answer all questions and are very helpful. My grandmother had to stay at Brookfield for about a month. Every single person who worked there was kind, respectful, courteous and seemed to genuinely have the patients well being as their goal. They were always smiling and would greet me when I came to visit. They took excellent care of my grandma and I would highly recommend this facility to anyone. Great nursing facility. Staff are very professional and compassionate. They tend to everybody from the moment you walk-in (even guests) until you leave. The administrator and/or director also helps. We've visited other facilities and this by far has been the most welcoming around the area. Thanks Brookfield!

Brookfield is by far the best facility I have ever walked into. The administrator David always has his door open and greets all the patients. There is no foul odor when I walked in. The receptionist gave me a tour of the facility and knew every patient by their first name. The patients are very involved in the activities, the entire table was full for the painting activity. All of the staff greets you with a smile and are very attentive. One of a kind building and service.

This place is mmaculate!!! It's a 6 star resort, not a 5! Staff is amazing all the way from the gal who mops the floor to the head boss. I've encountered every nurse and I've never ran across a bad one yet. They are always concerned about my better half! They keep you occupied with all kinds of enjoyment to keep your mind off the pain. It's my second time here. The rehab is very good. I love them all! They push me to do better for myself but they do not try to hurt me. They're very protective of me. I highly recommend this place!

I had the best experience at this nursing home. Cleanliness is up to par. You don't get a nursing home feel. I was greated by a loveley receptionist who knew the answers to my questions. The nurses were very attentive and answered the call light very quick. My grandmother was very well taken care of. I had the chance to meet the administrator David Howel and he is a genuinely nice person with a big heart. Along with the therapists; they are very focused on the patients and make therapy fun for them with their upbeat and well spirited personalities. I would definitely recommend this place.

Every one of these five stars has been well deserved. My mom was recently admitted to this facility & the care she has received has been over the top. To begin, I want to talk about the nurses. Their communication with the family members is not only consistent, but it is very helpful and kind. They are very attentive to their patients' needs & are very diligent. The facility is very clean, which is something we highly value. The therapists are also great at their craft, helping patients move at their own pace. Overall, everyone in this place is very good at what they do & I highly recommend it
