Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule. Payment is due the day of service, including copayments, coinsurance and any other charges. If your care isn't covered by insurance, you are entitled to a 25% discount.
We will do our best to work closely with your insurance company. However, it is your responsibility to know the details of your health plan with regards to co-payments, deductibles, referral guidelines and what services are covered under your plan.
