Services
Our goal is to provide comprehensive, state-of-the-art medical and surgical care for diseases of the ear, hearing, facial nerve, and auditory/vestibular nerves for both adult and pediatric patients in a friendly, efficient, and professional environment. We specialize in ear, nose and throat disorders in children, including performing ear tube surgery as well as tonsillectomies and adenoidectomies.
Our on-line forms are in PDF format. You may download our forms and fill them out prior to your visit. For new patients, please print the Registration Form and the appropriate Medical History Form for Adults or Medical History Form for Children < 12yrs. Please bring them with you to your office visit.
At this time, we do not participate with BCBS Partners, Cigna, Champus, Partners Medicare. As specified by your insurance company, co-payment and deductibles are due at the time of your office visit. We will file a claim form with your insurance company for you. If you are covered by an insurance plan that we do not participate with, we require payment in full at the time of service.
Prior to your operation, it is very important for you to avoid taking aspirin, aspirin containing, or aspirin-like medications such as ibuprofen, etc. These medications can cause both intraoperative and postoperative bleeding complications. You will find a link under the "Preoperative Instructions" to Aspirin products that should be avoided.
We are pleased that you have chosen us to perform your surgery. In order to help you get ready for your procedure or your child's procedure, we have included preoperatively instructions for your to follow for some of our common procedures. Bathing - If you are scheduled to have an ear operation, please shampoo your hair and wash carefully behind your ears the evening before your operation.
